Objective To investigate the methylation status of RIZ1and ID4gene promoter inpatients with different TCM syndromes of myelodysplastic syndromes (MDS), and tostudy the relationship of RIZ1and ID4gene methylation in different TCM syndromes ofMDS. Method The prognostic scores for35patients were calculated using theInternational Prognostic Scoring System (IPSS), the methylation status of RIZ1and ID4gene promoter in bone marrow mononuclear cells of35MDS patients and15patients withbenign hematological diseases was detected by methylation specific polymerase chainreaction (MS-PCR). Result (1) There was a significant correlation between traditionalChinese medical syndromes and IPSS prognostic scores (P<0.001, R=0.596). Patients withstasis toxin prevalence had worse prognosis than patients with spleen and kidneydeficiency.(2) The RIZ1and ID4genes were all completely unmethylated in bone marrowsamples from patients with benign hematological diseases as control. RIZ1and ID4genemethylation was found in15and12cases of35MDS respectively, which were higher thanthat in the control group (P=0.006vs P=0.01).(3) Different degrees of RIZ1genemethylation was found in different types of MDS, but there were no significant differencesamang them.(4) The positive rate of ID4gene in patients with stasis toxin prevalence wasmuch higher than that with vital qi deficiency, there was significant difference between thetwo group (P=0.011). Patients with marrow blast ratio beyond5%, as well as higher riskIPSS subgroups both exhibited a higher frequency of ID4methylation than that withmarrow blast ratio under5%and lower risk IPSS subgroups respectively (P=0.034vsP=0.030).(5) The coverage rate of RIZ1and ID4gene mathylation was57.1%(20/25), thepositive rate of both RIZ1and ID4gene mathylation was20%(7/35). Conclusion The IPSS prognostic scores are relative factors of TCM syndromes of MDS. Since theincidence of RIZ1methylation was not statistically different in different traditionalChinese medical syndromes of MDS, it is suggested that RIZ1methylation is a relativelyearly event. High expression of ID4gene methylation in patients with stasis toxinprevalence has verified the essence of evil domination in the advanced MDS at themolecular level, and provided objective evidence for the pathogenetic evolvement of MDSin TCM, and might has reference value in condition judgment and prognostic evaluation ofMDS. RIZ1and ID4gene methylation study may be helpful to provide reasonablearguments for the pathogenesis research, condition and prognostic evaluation, andintegrative medicine therapy in MDS.
