Objective:To establish the value of the rapid pathogens diagnosis of acute infectious conjunctivitis and mixed infections using multiplex polymerase chain reaction(MPCR).Method:Establish the MPCR system of standard strains of bacteria and pathogenic adenosis strains.131cases (131eyes)of clinical samples by clinically diagnosed as conjunctivitis to be detected by MPCR and with bacterial culture,of which typical acute bacterial conjunctivitis group (38eyes),the typical acute viral conjunctivitis group (35eyes), atypical acute conjunctivitis group (30eyes), atypical acute conjunctivitis group (30eyes), normal control group (28eyes), Take specimens from conjunctival sac and DNA was extracted by MPCR amplif and the results of gel electrophoresis were observed with the gel imaging analysis system ication.and the results of gel electrophoresis were observed with the gel imaging analysis system.Result:(1) DNA of the standard Staphylococcus epidermidis, Staphylococcus aureus, Escherichia coli, pneumonia streptococcal was amplified520bp specific bands in the MPCR system.(2)(Group Ⅰ)There were twenty-five eyes was amplified520bp specific bands in the typical acute bacterial conjunctivitis group (positive rate65.8%), significantly higher than (group Ⅱ seven eyes in the typical of acute viral conjunctivitis group (positive rate20.0%,)(X2=15.516, P<0.001); twenty-one eyes was amplified1004bp bands in the group Ⅱ (positive rate60.0%) was significantly higher than six eyes in group I (positive rate17.1%)(X2=17.071, P<0.001).(3) There were forty eyes was amplified520bp specific bands in the68eyes of group I and group Ⅲ (atypical acute conjunctivitis group)(positive rate58.8%), the positive rate of bacterial culture was67.6%, no significant difference between the two detection methods (X2=1.139, P=0.286).(4) Compared with the positive rate of MPCR in the group Ⅰ、Ⅱ、Ⅲ and Ⅳ (control group), the results were significant statistical difference (P<0.001), the MPCR detection rate of group Ⅰ, Ⅱ,Ⅲ was significantly higher than of group IV.(5) Compared with the positive rate between the MPCR bacterial detection rates and bacterial culture in group Ⅰ,Ⅱ,Ⅲ and IV, there was significant difference between the two detection methods in the group IV (X2=10.308, P=0.036). The detection rate of bacterial culture in the normal group is higher than the MPCR.Conclusion:(1) There was a significant difference bewteen the detection rate of acute bacterial and viral conjunctivitis by the MPCR, indicating that there was good specificity in the MPCR detection.(2) It has significant clinical application to carry out the pathogens diagnosis of acute infectious conjunctivitis useing the MPCR. MPCR is a rapid,sensitive,specific, simple method to compared with the bacterial culture method.
