Bjective: To observe the clinical effect and reoccurrencerate of combining fire-needling and Xiaocuo decoction in treatment ofaccumulation of phlegm and blood stasis. Methods:Divide90cases ofaccumulation of phlegm and blood stasis into three groups randomly.30cases of treatment group were treated by fire-needling with Xiaocuodecoction,30cases of control group were treated just by fire-needling,30cases of control group take ROM erythromycin orally,and D A sourcream and Clindamycin Phosphate Gel for external use. The test lastedfor4weeks. Before and after treatment, the three groups of cases weredone with symptom score about skin lesion and security evaluation oftherapy by following "Guidelines for clinical research of TraditionalChinese Drug Research (On trial) Acne efficacy criteria. Clinical effectwas examined after therapy lasted for4weeks and the reoccurrence ratewas indentified after2months of treatment. Results: Symptom scorecomparison of three groups before and after treatment: By T test,thesymptom score comparison of the treatment group is as following: thescore of before treatment is28.00±2.61,4.64±4.25for the aftertreatment, and the t=24.766,P <0.001,it shows a very significant difference; the symptom score comparison of the group with fire-needlingis as following: the score of before treatment is28.90±1.90,7.97±1.97for the after treatment, and the t=21.367,P <0.001,it shows a verysignificant difference;the symptom score comparison of the group withwestern medicine is as following: the score of before treatment is27.48±2.91,10.81±5.88for the after treatment, and the t=13.202,P <0.001,it shows a very significant difference. After4weeks of therapy, thecomparison of curative effect among three groups is as following: the28cases totally of treatment group:19cases were recovered,7cases weresignificantly effective,1case was effectual and1case was unfruitful withthe total cure rate was67.86%and effective rate was96.43%;29cases infire-needling control group:9cases were recovered,11cases weresignificantly effective,3case was effectual and6case was unfruitful withthe total cure rate was31.03%and effective rate was79.31%;27cases inwestern medicine control group:9cases were recovered,12cases weresignificantly effective,1case was effectual and5case was unfruitful withthe total cure rate was33.33%and effective rate was81.48%. Accordingto Chi-square test, X~2=6.10,P=0.045, the difference of effective rate inthree groups is statistically significant.2months after treatment,follow-up study was done to the recovered, significantly effective andeffectual cases. The recurrence rate comparison of three groups is asfollowing:4cases in treatment group recurred and the recurrence rate was15.38%;9cases in fire-needling control group recurred and therecurrence rate was7.50%;10cases in western medicine control grouprecurred and the recurrence rate was40.91%. According to Chi-squaretest of recurrence interval,X~2=6.038,P=0.049,the difference ofrecurrence rate in three groups is statistical significance. Adversereactions:5cases were shown up in western medicine control group,themain symptom was local-dry-skin,desquamation and gastrointestinaldiscomfort; both of fire-needling control group and treatment group have2cases, main symptom was local pain. All the adverse reactions areminor ones, which were vanished by giving gastric mucosa protectionmedicine, moisturizing your skin or cold compresses. Conclusion: Fourweeks later, by comparing with fire-needling group and western medicinegroup, the clinical effect of treatment group was obviously better than thetwo other control groups with P=0.045and the difference is statisticalsignificance; symptom score was done about types, amount, swelling,colors of skin lesion and skin oil of the three groups of cases before andafter treatment, P<0.001, which shows that there is significantlydifference of three groups before and after treatment; follow-up study wasdone to significantly effective cases in three groups two months aftertreatment, the recurrence rate of the treatment group is significantly lowerthan the two other groups with P=0.049and the difference is statisticalsignificance. Therefore, the treatment of fire-needling with Xiaocuo decoction has great effect on accumulation of phlegm and blood stasis.Because of the better effects, low recurrence rate, few side-effects of thiscombined method, it is fit for clinical promotion.
