Objective: To review and analysis of the HIV/AIDS infection patients with whomcentral nervous system lesions in the clinical manifestations, biochemistry, immunology,cerebrospinal fluid routine, etiology, head computed tomography (CT) or magneticresonance imaging (MRI) and brain biopsy and other tests, to improve the diagnosis rateof these person. Methods: To summarize51patients with AIDS combined with centralnervous system disease whom in our hospital for treatment from January2007toDecember2011clinical data retrospectively. Results: Among354cases of HIV/AIDSpatients,51cases have central nervous system lesions, the incidence rate is14.4%.Tuberculous meningitis18cases (35.3%), with rapid onset, mainly for tuberculosispoisoning symptoms, headache, nausea, vomiting, disorder of consciousness. after therapy,fatality rate as high as33.3%; Toxoplasma encephalopathy4cases (7.8%), Mainly asfever, fatigue, weight loss, headache, nausea, vomiting, some person with disturbance ofconsciousness;22cases of the virus-associated encephalopathy (43.1%),9cases ofsymptomatic patients with slow onset, mainly memory loss, may have personality changes,1case show brainstem encephalitis, mainly fever, disturbance of consciousness;cryptococcal meningitis1case(2.0%), acute onset, mainly as headache, nausea, vomiting.Brain atrophy in9cases (16.7%), Among51patients,33cases have clinical symptoms,23cases improved after treatment,8cases discharged, and5cases death.36cases acceptedof the ART treatment,24cases show that the treatment is effective,5cases died, referralin1,6cases of lost to follow-up.48cases do the CD4/CD8check, CD4+>200/mm3in8cases (16.7), CD4+<200/mm340cases (83.3%) and CD4+<100/mm326cases(54.2%).1cases of clear etiological basis, the rest are the clinical diagnosis. Conclusion:HIV/AIDS patients is easily complicated by a variety of central nervous systemcomplications, diagnostic are difficulties and with high mortality. For AIDS patients, especially whom CD4counts<200/mm3, if he/she has the above symptoms, it is timelyto check the cerebrospinal fluid and cranial imaging check, line the brain biopsy whennecessary, do our best to get etiology and pathology support to improve the diagnosis ratefor timely treatment and improve prognosis.
