Objetive: To compare the efficiency of the hepatic resection andtransarterial chemoembolization (TACE) for patients with hepatocellularcarcinoma (HCC), stage B in the Barcelona Clinic Liver Cancer(BCLC)classification.Methods: Between January,2003and August,2006,191patients withBCLC stage B, Child pugh classification A HCC were included in thisretrospective study. Different statistical methods were used to compare thesurvival rate of the two groups. The risk factor was analyzed by the Cox’sregression model. The overall survival rate was analyzed by the Kaplan–Meiermethod.Results: Of these191patients,105patients underwent hepatic resectionand86patients received TACE. The1,3,5-year overall survival rates for the two groups after hepatic resection and TACE were69%,49%,38%and50%,22%,8%, respectively; median survival time were38.3month and16month(P<0.05). The Cox regression model analysis showed that higher AFP Levels(>400ug/ml) and the treatment method(TACE) were independent predictorsof poor survival rate.Conclusions: Hepatic resection for BCLC stage B, Child-pugh A HCCpatients might had better survival rates than TACE. Different subgroup of BCLCstage B HCC patients could be received different treatment strategy.
