Clinical Analysis Of The Characteristics Of No.12、13Group Lymph Node Metastasis In Non-small Cell Lung Cancer

Objective:To investigate the clinical value and characteristics of No.12、13group lymph node metastasis in non-small cell lung cancer.Methods:141patients with non-small cell lung cancer underwent pneumonectomy or lobectomy and lymphadenectomy. The law of lymph node metastasis was analyzed. This study selected a total of141patients with non-small cell lung cancer underwent pneumonectomy or lobectomy and lymphadenectomy in the period from October2010to October2011in Tianjin Chest Hospital.Naruke’s map was taken as a reference of sign of thoracic lymphadenectomy and drawing materials.exploration and dissection of1～11group lymph nodes in accordance with this standard, No.12、13group lymph node which in postoperative specimens was drawn and marked. TNM staging was confirmed after pathological analysis of all lymph nodes.Then based on the results to study the general situation of positive frequency of No.12、13group lymph node and observe the relationship between lymph node metastasis and size、 location、differentiation degree and pathological type of primary tumor.Because the drawing materials of No.12、13group lymph node has some difficulties and part of the location of these two groups lymph nodes are very close, this article will take No.12、13group lymph node as a whole objec.Pathological types according to the WHO classification, TNM staging according to the UICC2009latest revised standard.Results:A total of2154lymph nodes were resected and the mean number of dissected lymph nodes was15.3. Positive frequency of lymph nodes was13.1%.55patients were confirmed to have thoracic lymph node metastasis in141patients with non-small cell lung cancer and the metastatic rate was39.0%.The number of patients of N1,N2and skipping N2was15,28and12. Seven patients whose metastasis of lung cancer only involved No.12、13group lymph node were found in93patients without lymph node metastasis and the detection rate was7.2%.The metastatic rate of No.12、13group lymph node in T2stage was obviously higher than that in T1stage (P<0.05),the metastatic rates of No.12、13group lymph node in poorly and moderate differentiated lung cancer were25.0%and8.9%respectively (P<0.05),The metastatic rate of No.12、13group lymph node of peripheral lung cancer was much higher than that in central lung cancer (P<0.05)Conclusion:(1) The positive frequency of No.12、3group lymph node was9.8%andl2.4%.There was no significant difference of positive frequency between No.12、13group lymph node and other group lymph nodes.lt was a kind of meaningful index parameter.(2) Seven patients whose metastasis of lung cancer only involved No.12、13group lymph node were found in93patients without lymph node metastasis and the detection rate was7.2%.Pathological examination of No.12、13group lymph node of early stage non-small cell lung cancer may provide directive value for clinical staging of non-small lung cancer.(3) The frequency of metastatic rate of No.12、13group lymph node significantly correlate with size of primary tumor, pathological classification and cell differentiation.
