Objective:To probe the difference between ultrasound elasticity classification in lymph node metastasis group and in none lymph node metastasis group of breast cancer. And to discuss the correlation of ultrasound elasticity classification and the microvessel density in breast cancer.Methods:One hundred breast cancer patients were selected in the breast and thyroid surgical department of ZunYi Medical College Affiliated Hospital during March2010and October2011. In all patients, regular ultrasound and elastic ultrasound was performed to evalue the rigidity of the lesions and the axillary lymph node were be detected the size, number, rigidity by using the Sonoline scanner and a10-MHz linear probe (Siemens Acuson Antares5.0Medical Systmes) before operation. divided the one hundred patients into lymph node metastas is group and none lymph node metastasis group according to the result of pathological examine. Each group has fifty cases respectively.The microvessel density was measured by immunohist ochemical staining of their formalin-fixed paraffin-embedded tissue with monocloal antibody CD31. The difference of ultrasound elasticity classification in two groups and the con-elation between the grades of ultrasonic elastography and MVD were analyzed.Meanwile the diagnosis value of ultrasonic elastography in the same axillary lymph node metastasis was analyzed according to the result of the pathological examine.Results:1.most cases were classified for the3,4and5grade by ultrasonic elastography. Among the one hundred cases, there were zero case in grade one, two cases in grade two. sixteen cases in grade three, forty-four cases in grade four and thirty-eight cases in grade five. Fifty none lymph node metastasis group were compounded by three case in grade three, twenty-one in grade four and twenty-six in grade five, Fifty lymph node metastasis group were compounded by two cases in grade two, thirteen in grade three,twenty-three in grade four and twelve in grade five. The difference of ultrasound elasticity classification in two groups was obvious (p<0.05).2.Four hundred and sixty-eight lymph nodes were detected in the one hundred patients and among them fifty-eight metastasis nodes were suggested by ultrasound before operation.Nine hundred and fifty-five nodes were found after operation.forty-three metastasis nodes were found after operation.Forty-three metastasis nodes were confirmed among fifty-eight nodes indicated by ultrasound.The other fifty nodes were reactive hyperplasia.Fourty-three metastasis nodes were compounded by four cases in grade one,six in grade two,fourteen in grade three, eleven in grade four and eight in grade five; the fifteen no metastasis nodes were compounded by five in grade one, six in grade two, three in grade three, one in grade four and zero in grade five.3.The differences of MVD was significantly in individual patients. Most patients have1-3hot spots, some cases even have six hot spots. But some cases have less hot spots, some even can’t find hot spots, Sparse brown blood vessels are visible under high magnification (×200),And some cases have more hot spots (average3-4), The microessel-density counted higher (up to78).The average microessel-density of the fifty cases of lymph node metastasis patients were12-78, ranged (38.02±14.88). The average microessel-density of the fifty cases of none lymph node metastasis patients were7-45,ranged(28.16±11.48),The microessel-densit y counts of patients of the two groups are significantly different. This is statistically significant (P<0.05).4.Two cases were grade two in the Elasticogrhpy classification.The average microvessel-density counted (41.5±12.02). Sixteen cases for grade three and the average microessel-densi ty counted (37.5±16.75). Fourty-four cases for grade four and the average microessel-density counted (33.68±12.55).Thirty-eight cases for grade five and the average microessel-density counted (30.11±14.53). ultrasound elastography classification have some correlation with MVD (p<0.05,F=4.41).Conclusion:l.The ultrasound elastography was of great value in the diagnosis of breast cancer.2.Most cases of none lymph node metastasis were four or five grade in elastography, most cases of lymph node metastasis were three or four or five grade in elastography.The difference of ultrasound elasticity classification in two groups was significant (p<0.05);3. Ultrasound elastography is valuable in clinical stages of breast cancer.4. LN(+) group MVD value is greater than LN (-)(P<0.05). The count of MVD decrease (P<0.05) while the classification of elastic increases in breast cancer. ultrasound elastography classification have some correlation with MVD (p<0.05,F=4.41).
