ObjectivesTo study the mode of individual staged holistic nursing on spinal cord injurypatients with neurogenic bowel disfunction and its influence on the patients’ bowelfunction and quality of life.Methods30patients in our hospital SCI medical ceter of Orthopaedics department Ⅰfrom Juneto September,2011were chosen into intervention group.32patients in the ward ofOrthopaedics department Ⅱwere chosen into control group in the same period.Theintervention group get bowel management with the mode of individual staged holisticnursing.The control group just get routine bowel nursing.We get the scores about thebowel function and quality of life by the questionnaire of The International BowelFunction Basic SCI Data Set、The present International Bowel Function Extended SCIData Set、Wexner Continence Grading Scale、WHOQOL-BREF on admission、acutephase4weeks after injury、inpatient rehabilitation phase eight weeks after injury and threemonths of follow-up phase after the discharge.At last contrast the scores in the two groups.Results1、After intervention, the bowel functions of the two groups in acute phase andinpatient rehabilitation phase were all improved (P<0.05), but the intervention groupimproved more evident;in follow-up phase,the bowel functions of intervention group gotsignificant improvement (P<0.05), but the control group got a downturn.2、Comparing intervention group with control group,the functions of two groupshad significant difference in acute phase、inpatient rehabilitation phase and three months offollow-up phase(P<0.05).3、After intervention, in acute and inpatient rehabilitation phase the total score、 Physical domain score and enviromental domain score of two groups were higher thanthose on admission(P<0.05). The psychological domain score was lower(P<0.05)andthe social domain score was no significant difference(P>0.05).In follow-up phase,the totalscore and the4domains’ score of QOL in intervention group all elevated than those onadmission.On the contrary,the total score and the4domains’ score in control group alldecline(P<0.05).4、After intervention, in acute and inpatient rehabilitation phase the total score and thephysical、 psychological and enviromental domain score all elevated than controlgroup,the difference was significant(P<0.05). and the social domain score was nosignificant difference(P>0.05). In follow-up phase,the total score and the4domains’ scoreof QOL all elevated than control group, the difference was significant(P<0.05).ConclusionsThe f individual staged holistic nursing on spinal cord injury patients with neurogenicbowel disfunction can help to reduce bowel complications、resume the bowel function、elevate patients’ quality of life. the mode of individual staged holistic nursing can beapplied to practice..So the results of this research can provide theoretics support for nurses.
