| “Dushen Soup†is a prescription composed only of Panax Ginseng, and wasused to treat vomiting blood, vaginal bleeding, and blood halo etc. after sweating ordiarrhea in the ancient. Now it is often used in emergency and accident, especially inpostpartum hemorrhage. For the patients with postpartum hemorrhage, promotingthe shrink of uterus is an usual method to stop bleeding, such as the usage ofGong-xue-ning or oxytocin. But when the patient is in heavy bleeding and extremelyweak constitution, oxytocin usually can not work. In this situation, Dushen Soup hasbetter effects against hemorrhage than oxytocin. According this, the project focuseson the researching of active ingredients which has the effect of uterus shrink inDushen Soup, with the aim of exploring the mechanism of Dushen Soup againstpostpartum hemorrhage.This research tried to get the major saponin components from Dushen Soup,with the effects of strengthen uterus smooth muscle shrink and hemostatic.Afterwards, the effect of influence intracellular Ca2+ concentration by thesecomponents was investigated to make clear the relationship between promotinguterus smooth muscle shrink and the change of intracellular Ca2+ concentration,which is related to the hemostatic mechanism of Dushen Soup. The results of thisresearching will be benefit of using Dushen Soup more safely and effectively inclinic, and Ginseng study deeply.Red ginseng was boiled to get Dushen Soup, and then macroporous resin wasused to obtain the total saponin.7compounds were isolated by the positive phase andreverse phase chromatography column, HPLC, and recrystallization. Their chemicalstructure were identified by NMR spectral analysis as20(S)-protopanaxadiol(PPD),20(S)-ginsenoside Rh2, ginsenoside Rb1, ginsenoside Rc,20(S)-ginsenoside Rg3, ginsenoside Re, andginsenoside Rg1respectively.7components separated from Dushen Soup were tested the effects of thenonpregnant female rats uterus smooth muscle shrink. The results showed thatginsenoside Rg1and Rb1inhibited uterus smooth muscle, in contrast, ginsenoside Repromoted uterus smooth muscle shrink, but ginsenoside Rc, Rh2, Rg3andprotopanaxadiol had little effect on uterus smooth muscle. Because the concentrationof ginsenoside Re was more than total of Rg1and Rb1in Dushen Soup, it showed thefinal effect of strengthening uterus shrink, and when used in large quantum it wouldprevent uterine bleeding.Ginsenoside Rg1, Rb1, and Re were test their effect on the concentration of intracellular Ca2+, the results show that ginsenoside Rb1and Rg1decreasedintracellular Ca2+ concentration, but ginsenoside Re increased the intracellular Ca2+ concentration. In physiological condition, the increase of intracellular Ca2+ was themainly reason for smooth muscle shrink, so the results suggest that Ginsenoside Re’seffect of increasing intracellular Ca2+ caused to promote uterus shrink, but Rb1andRg1decreasing intracellular Ca2+ is related to inhibit uterus smooth muscle shrink.From this researching, the conclusion can be made that ginsenosides in DushenSoup have two-way adjustment on intracellular Ca2+ concentration, some of themincrease intracellular Ca2+ concentration, others decrease intracellular Ca2+ concentration, so Dushen Soup shows two-way adjustment effect on uterus shrink,which change with Dushen Soup concentration and in vivo metabolism. |
