Background and objective:To explore the simple posterior cranial fossa decompression and atlantooccipital fascianeurolysis and decompression of posterior cranial fossa and the cerebellar tonsillarherniation resection after curative effect and postoperative complications, to provide help forthe clinical treatment of Arnold-Chiari I malformation selection operation treatment.Methods:We have collected40patients treated by operation with Chiari I malformation in ourhospital from2010May to2012December,, and recorded the gender, age and history,through the CT, MRI imaging and clinical diagnosis, using two different operation approachto compare the efficacy of Arnold-Chiari I malformation during the operation and afteroperation postoperative complications. The acquisition cases randomly assigned to A, B twogroups, observing group20(Group A): treated by posterior cranial fossa decompression andatlantooccipital fascia neurolysis,20in the control group (Group B): by posterior fossadecompression and resection of the cerebellar tonsillar hernia. Operation and diagnosisstandard: head and neck junction MRI sagittal, big hole in T1like the cerebellar tonsillar tipto the occipital, the cerebellar tonsillar hernia into the foramen magnum are lower than5mm,a minimum of third cervical centrum lower edge. Finally,we try to find the ideal surgicalmethod after statistical analysis.Results:1.Comparing the effect of operation treatment of Group A and Group B, all the clinicalsymptoms of the patients in the hospital were improved significantly or disappeared.Nodeath case. Patients were followed for one year, no recurrence. The postoperative curativeeffect had no significant difference (P>0.05). 2.Postoperative complications in Group B were significantly increased than Group A, Therewas statistically significant (P<0.05). Conclusion postoperative therapeutic effect of Group Awas better than that of Group B.Conclusion:Through the research group, simple posterior cranial fossa decompression andatlantooccipital fascia lysis in the treatment of Arnold-Chiari I malformation is an idealsurgical method, this method can not only get better treatment effect but also can reduce theincidence of postoperative complications,It is worthy of promotion in future clinical work.
