Objective: To evaluate the effect of neoadjuvant therapy of anusreservation operation for ultra-low-rectal carcinoma.Method: From January2005to September2011, patients wereselected with ultra-low-rectal carcinoma in our medical center.Thegender,age,location and size of the tumor,degree of differentiation,Dukes stage,operation methods,preoperative neoadjuvant therapy,andwhether or not retainning the anus before operation were studied throughLogistic regression analysis.These patients were divided into2groups.42patients who received neoadjuvant therapy were divided into A group,the other42patients who only received direct surgical treatment weredivided into B group.Results: All cases in group A got the operation,12by Dixon,5byHartmann and25by Mile’s,the annus-retained rate was28.5%. Ingroup B,2cases operated by Dixon,6by Hartmann,25by Mile’s,9byproximate colonstomy or ileostomy,and the annus-retained is4.8%.During the12-48months followed up,The local recurrence rate was11.9%in group A and42.9%in group B.The survival rate of3years was95.2%in group A and69.0%in group B.Conclusion: Rectal neoadjuvant therapy can improve theultra-low-rectal carcinoma rate, the survival rate of3years, and reduce the local recurrence rate.
