Abstract/Summary:
The Human adenovirus (ADV) is one of the important pathogens of acuterespiratory infections in children,this study is to understand the Changchunarea from February2012to March2013pediatric acute respiratory infectionsin children with human adenovirus (ADV) infection, ADV type and clinicalcharacteristics were analyzed.We collected1329cases of throat swab specimens from February2012toMarch2013, acute respiratory infections children in my hospital in ChangchunCity, ADV detection with fluorescence quantitative polymerase chain reaction(PCR) method were positive for140cases, were we analysis ADV genotype inthe140positive DNA respectively through1,2,3,4,5,7,11and21type eightpairs genotyping primers, and different clinical condition. ELISA method wasused for the qualitative detection of100positive specimens of plasma of infantin anti-adenovirus IgG and IgM antibodies, and selected also100cases ofnon-ADV-infected children are as controls.Results: In our hospital in1329were respiratory specimens, fluorescencequantitative polymerase chain reaction (PCR), to determine the ADV-positivespecimens for140,the positive rate was10.53%,results of DNAPCRgenotyping of140positive specimens:91cases determine the types by1,2,3,4,5,7,11and21typing and primer amplification: type1ADV12cases,accounting for8.57%(12/140),type2ADV26cases, accounting for18.57%(26/140),type3ADV3cases, accounting for2.14%(3/140), type4ADV32cases, accounting for22.85%(32/140),the type7ADV5cases,accounting for3.57%(5/140),21type ADV13cases,accounting for9.28% (13/140),49cases by genotyping primers amplified were negative.Clinical expression: The observation group were100positive specimen ofanti-adenovirus IgM antibodies of ELISA method for the qualitative detectionin the children plasma,and100cases of non-ADV-infected children of IgManti-adenovirus antibody in plasma were selected as a control group. In theobservation group in the58cases, were positive detection rate of58%.Thecontrol group was58%of positive detection rate in58cases of IgM antibodiesof anti-adenovirus。The observation group than the control group by theχ2test,P <0.05,it suggest that antibodies of anti-adenovirus have a higher levelsadenovirus PCR positive samples in plasma. ELlSA detect IgM use for thediagnosis and differential diagnosis of adenovirus in acute respiratoryinfections.Anti-IgG antibodies by ELISA method use for the qualitative detectionin the plasma of100positive specimens children as the observation group than100cases of non-ADV-infected children plasma as a control group.Theobservation group of anti-adenovirus IgG antibodies in the number of53cases,the positive detection rate was53%.Control group of anti-adenovirus IgGantibodies in the number of42cases, the positive rate of42%. The observationgroup than the control group by theχ2test, P>0.05, no difference to the IgGanti-adenovirus antibody in the observation and control groups, IgG antibodyhad a higher average.140positive case including82males and58women, agerange1month to13years of age. ADV detected positive children in21cases of acute upperrespiratory tract infections, pharynx, the conjunctivitis hot5cases, acutebronchitis,39cases of140patients with acute respiratory infections, bronchialpneumonia,64cases,11cases of bronchiolitis. The study common symptoms:fever,89cases (63.57%), cough,126cases (90%), breathlessness and62 cases (44.29%), angina pain in20cases (14.29%), rhinorrhea in32cases(22.85%), conjunctival hyperemia6cases (4.29%), lymph nodes in10cases(7.14%), polyuria two cases (1.42%), diarrhea in52cases (37.14%), rashin5cases (3.57%), headache, vomiting three cases (2.14%). Commonsigns: lung wet and dry rales in79cases (56.42%), the pharynx herpes in22cases (15.71%), and conjunctival hyperemia in6cases (42.8%), swollenlymph nodes in10cases (7.14%), five cases of rash (3.57%) Laboratoryexamination: leukopenia17cases (12.14%), increased the proportion ofperipheral blood lymphocytes of123cases (87.86%), thrombocytopenia(2.14%), pancytopenia (1.43%),13cases of abnormal liver function(9.29%), cardiac enzyme changes in71cases (50.71%), and change the X-ray imaging in107cases (76.43%), organ multi-system damage in3cases(2.14%).The final diagnosis: acute upper respiratory tract infection in21cases (15.00%), pharynx, conjunctivitis heat in5cases (3.57%),39cases(27.85%) of acute bronchitis, bronchial pneumonia,64cases (45.71%),11cases of bronchiolitis (7.86%). All patients were cured after active treatment.This study suggest that the Changchun region had a ADV infection, ADVinfection rate was10.53%, the high season for spring and summer cross season,summer, autumn, autumn and winter cross-quarter, men and women ratio of1.41:1, in February2012to March2013. ADV mainly caused from acuterespiratory infections (79.6%) of the children under five years of age. ADV cannot only cause tonsillitis, the herpes angina Yandeng the acute upperrespiratory tract infections, also cause bronchial pneumonia, bronchiolitis, lobarpneumonia and other severe lower respiratory tract infections, accordance toforeign reports results, should be pay attention. ELlSA detection of IgM can beused for diagnosis and differential diagnosis in plasma in patients ADVinfection and non-ADV-infected children. Higher IgG antibodies indicating that there is a certain resistance to adenovirus, but in recent years, the studieshave shown that adenovirus recombination occurs, should be brought to ourattention.
