A Control Study Of Acupuncture Suggestion Therapy Combined With Antidepressant Drug Treatment Of Somatization Disorder

Somatization disorder, also known as Briquet’s syndrome, somatoform disorders,one of the most common clinical subtype,more common before the age of30, and in women.The course of the disease lasts for at least2years,mainly shows as repeated,varied,frequently changing physical symptoms,somatic symptomsin almost every system.No suitable physical illness can be found to explain patients’symptoms, often lead patients repeatedly to seek medical treatment, resulting in a certain serious social dysfunction.Somatization disorder, by definition, is an impact on multiple systems with a variety of symptoms of chronic mental disorders.Course of the disease was chronic volatility,patients often have many years of treatment experience,a large number of auxiliary examination data, using a variety of drugs.even repeatedly undergo unnecessary surgery, resulting in enormous medical and economic costs. Somatization disorder differs across cultures. Compared to Western society,people innon-Western societies are more prone to have somatization disorder.Etiology and pathogenesis of somatization disorderis not very clear, and its exact mechanism is still under further study.The prevailing view was that the cause of somatization disorder is the result of multiple factors, including genetics,personality characteristics, family rearing patterns,neuropsychological and neurophysiological. psychological and social factors, etc.Given so many factors that cause somatization disorder,the current treatments of the disease are mainly in four aspects:the medication,psychotherapy,drug combination of psychotherapy and other treatment. An effective drug treatment is new antidepressants,such as Selective Serotonin Reuptake Inhibitors(SSRIs), Serotonin and Noradrenergic Reuptake Inhibitors (SNRIs). There are also studies confirmed that the small doses of an untypical antipsychotics can be a catalyst in curing somatization disorder;most commonly used psychological treatment is cognitive behavioral therapy techniques, including in dividual and group therapy.Moreover,Morita therapy, relaxation training, physical therapy,and treatment by Chinese medicine has gradually been applied to somatization disorder treatment.Psychotherapy has been recognized as important in the treatment of somatization disorder. However, based on past experience, there are several shortcomings of coginitive behavior therapy, such as the long course of cognitive behavior therapy treatment cannot be effective immediately, and it is difficult for patients with low educational level to accept it. Also,patients with somatization disorder often refuse to communicate with doctors to explore their psychological reasons,even those who have significant depression and anxiety. Therefore,the development of suitable local psychological treatment which considers patients’ characteristics,and is easy to accept and to operate becomes one of the important issues in somatization disorder studies.This study considered the characteristics of patients with somatization disorder, including their low education level,their easiness to be implied and their eagerness to solve the somatic symptoms,utilized traditional acupuncture as media, and the language of hint treatment method,and combined with antidepressant medication to treat patients with somatization disorder. We used Symptom Checklist90and objectivity of the event-related potentials to assess their effectiveness.Acupuncture implied therapy is that when patients receive the acupuncture treatment and implied language,the special organization in their cerebral cortex and subcortical becomes active,in this state,patients’attention dispersion to the external enviornment decreases,and their attention mainly concentrated in the implied language of the psychotherapist.Under the guidance of the therapist’s selective imply, patients’original concern of physical discomfortis turned into positive feelings of body,thus changing patients’awareness of the disease and improve their clinical symptoms.ObjectiveBy using a comparative observation of the clinical efficacy of acupuncture implied therapy combined with antidepressant drug treatment of somatization disorder and drug treatment alone, this study is to explore the supporting role of acupuncture suggestion therapy in the treatment of somatization disorder,and thus to provide a new treatment for clinical somatization disorder frame of reference.MethodsWe select a Hospital mental sleep section diagnosed107cases of patients with somatization disorder. They were randomly divided into the duloxetine and acupuncture implies treatment group (experimental group,referred to as acupuncture and medicine group)(N=54) and pure duloxetine treatment group(control group,referred to as the pure drug group)(N=53).The observation time is four weeks.Before treatment and4weeks after treatment, the clinical efficacy and the improvement of patients’cognitive function were accssed by Symptom Checklist90(SCL-90),event-related potentials (ERPs).SPSS13.0statistical software was used to analyized the results to examine the effect of acupuncture therapy combined with antidepressant therapy treatement for somatization disorder.Results1、Compared to Chinese population norm,the total score, the total average score and factor scores of SCL-90of patients with somatization disorder were significantly higher, and the difference was statistically significant(P<0.05).2、For the pure drug group,the SCL-90total score,total average score and factor scores after treatment were significantly lower than before treatment, and the difference was statistically significant(P<0.05);event-related potential P300,latency of N2after treatment and P3latency was significantly lower than before treatment, and the difference was statistically significant(P<0.05);N2amplitude after treatment,P3 amplitude significantly higher than before treatment, and the difference was statistically significant(P<0.05).3、For acupuncture and medicine group,the SCL-90total score,its total average score and its factor scores after treatment were significantly lower than before treatment, and the difference was statistically significant(P<0.05);event-related potential P300,latency of N2after treatment, and P3latency was significantly lower than before treatment, and the difference was also statistically significant(P<0.05);N2amplitude after treatment,P3amplitude significantly higher than before treatment (P<0.05).4、The SCL-90each factor score accessment:After treatment,the SCL-90total score,its total average score and its factors scores of acpuncture and medicine group were significantly lower than those for the pure drug group, and the differences were statistically significant(P<0.05);event-related potentials P300,Acupuncture latency of N2after4weeks’treatment, P3latency of the acpuncture and medicine group were all significantly lower than those of the pure drug group(P<0.05);acupuncture and medicine group N2amplitude after4weeks’treatment,and their P3amplitude were both significantly higher than those for the pure drug group,and the differences were statistically significant(P<0.05).5、The discrepancy of factor scores of SCL-90between the acupuncture and medicine group and the pure drug group from biggest to smallest were:somatization, and anxiety(tied),depression, obsession, terror, paranoia, psychosis,interpersonal sensitivity and hostility(ties).Conclusion1.In addition to a variety of somatic symptoms, patients with somatization disorder also have many psychological problems in mood,interpersonal relationships, coercion,terror, paranoia.The SCL-90can be a comprehensive assessment of patients with somatization disorder and its clinical symptoms.2.Single expense of duloxetine treatment of somatization disorder can significantly improve patients’physical symptoms and cognitive function. 3.Acupuncture suggestion therapy combined antidepressant drug treatment of somatization disorder suggested that in addition to the role of antidepressants, acupuncture suggestion therapy also improved patients’cognitive function, shifted patients’ attention on their somatic symptoms,and changed their unreasonable belief and distorted perception.4.Acupuncture suggestion therapy joint antidepressant drug treatment for somatization disorder was significantly more effective compared to single antidepressant therapy.5.Acupuncture suggestion therapy is a treatment that cater to the psychological characteristics of Chinese patients with somatization disorder, which can significantly improve the clinical symptoms of somatization disorder andpatients’cognitive function.6.The event-related potentials P300can be used as an objective index to evaluate the cognitive function of somatization disorder.
