Objective:To investigate the relationship among the intima-media thickness(IMT),serum uric acid (SUA) and intercellular adhesion molecule-1(ICAM-1) of type2diabetes mellitus(T2DM) with carotid atherosclerosis(CA), the thesis evaluates the effects of SUA and ICAM-1on atherosclerosis(AS).Methods:80cases with T2DM were tested by American SONOS5500ultrasonic color Doppler diagnostic instrument.According to IMT and the situation of spots in carotid artery, those80cases were divided into two groups: with carotid atherosclerosis (CA, n=40) and no carotid atherosclerosis(NC, n=40). The serum levels of SUA, ICAM-1, systolic blood pressure(SBP), diastolic blood pressure (DBP), fasting plasma glucose(FPG), fasting insulin(FINS), glycosylated hemoglobin Alc(HbAlc), two hour plasma glucose (2hPG), two hour insulin(2hINS), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C),total cholesterol(TC), triglyceride (TG) and carotid intima-media thickness(IMT) were detected. At the same time, body mass index (BMI), and homeostasis model assessment of insulin resistance (HOMA-IR) were calculated. By Pearson CA and correlation analysis of carotid artery IMT and the various clinical parameters correlation effect, further using the multivariate linear stepwise regression analysis to reveal the carotid artery IMT independent factors, distinguish its primary and secondary effects.Results:①The two groups in gender, age, duration of diabetes, diastolic blood pressure (DBP) comparison, the difference was not statistically significant (P>0.05), CA group of patients with BMI, systolic pressure (SBP) than that in NC group (t=2.07,2.18, P<0.05for all).②Patients in the CA group TC, LDL-C, TG, FPG,2hPG, FINS,2hINS, HbAlc, HOMA-IR higher than that of NC group (t=3.05,3.01,2.58,2.36,2.19,2.54,2.49,2.31,,2.63, P <0.01or P<0.05).③Two groups of patients with carotid IMT, SUA, ICAM-1shows, group CA patients with left, right carotid intima-media thickness average (C-IMT), SUA, ICAM-1was significantly higher than that of group NC (t=4.38,3.45,3.39, P<0.01for all).④The correlation analysis showed that Pearson:CA group of patients with C-IMT and SUA, ICAM-1, age, course, SBP, DBP, BMI, TC, TG, LDL-C, FPG,2hPG, FINS,2hINS, HbAlc, HOMA-IR were positively correlated (P<0.01or P<0.05), and negative correlated to HDL-C (P<0.05).⑤The group of patients with CA C-IMT as dependent variable, the detection of various clinical parameters as independent variables, multivariate linear stepwise regression analysis demonstrated that entered the regression equation is SUA, ICAM-1, TG, LDL-C, HOMA-IR, BMI, HbAlc, C-IMT and SUA tips patients in group CA, ICAM-1, TG, LDL-C, HOMA-IR, BMI, HbAlc positive correlation. Conclusions:①Type2diabetes individuals with atherosclerosis are characterized by metabolic disorder, insulin resistant, vascular endothelial dysfunction and inflammation.②SUA, ICAM-1, LDL-C, TG, HOMA-IR, BMI and HbAlc,which are listed by their weaker effects to C-IMT, are the most influential factors that affected C-IMT levels in type2diabetes individuals with AS. To sum up, the increasing of SUA, ICAM-1, insulin resistant, lipid disorder, and long-term poor controlling of blood glucose are all participated in the formation and progress of atherosclerosis.
