Objective To evaluate the safety and efficacy of paclitaxel-eluting sent in treatment ofacute coronary syndrome of elderly patients by the way of contrast.Methods120patients (86males and34females,mean age68.22±6.19years) wereenrolled in study. All patients received coronary angiogram and PCI.60patients(47malesand13females,mean age68.63±5.21years) were implanted with rapamycin-elutingcoronary stent(EXCEL) and60(39males and21females,mean age67.42±6.69years)patients were implanted with paclitaxel-eluting sent(YINYI). The patients in both groupswere similar with respect to age, gender, history, the segment of lesion, the length anddiameter of lesion. All patients were on an oral medication of300mg of aspirin andclopidogrel at the first time three days before PCI, then after300mg continue used for onemonth, aspirin were take100mg q.d infinitely, clopidogrel were used75mg q.d post-PCI.Both major cardiac events and restenosis were observed at6months’follow-up.Results The success rates of two groups were both100%, Among all97target lesions inYINYI group,77lesions were type B2/C complex lesions(79%)。The mean length of thetarget lesions were18.1±4.2mm. The degree of coronary artery stenosis were86.54±10.76%,The mean stent length and diameter were24.02±6.59mm and2.91±0.35mm. Among all101target lesions in EXCEL group,83lesions were type B2/Ccomplex lesions(90%).The mean length of the target lesions were17.2±6.1mm. The degree of coronary artery stenosis were85.79±11.12%.The mean stent length and diameter were23.00±6.11mm and3.02±0.28mm.Except1patient in YINYIgroup who dead with non-cardiac in the4th months, the remaining patients had completedthe clinical follow-up, mean follow-up (6.03±0.17) months.11patients in YINYI groupand14patients in EXCEL group were checked by angiography. YINYI group had3casesand EXCEL group had2cases of coronary angiography confirmed In-stent resrenosis(ISR),which were successfully implemented the TLR.In YINYI group and the EXCELgroup,6months MACE rates were5.0%and3.3%(P>0.05), no significant difference. Thetwo groups were not found with paclitaxel, sirolimus and its polymer-related adversereactions.Conclusion Paclitaxel-eluting sent is safe and effective in clinical application. It couldprovide similar outcomes as rapamycin-eluting coronary stent. The two sorts of stents canobtain satisfactory clinical results in the aspects of lowering MACE,reducing ISR andin-stent thrombosis, and there were no significant differences between them. But long-termclinical effects need to next follow-up observation.
