Objective:to compare the clinical evaluation in diagnosis of thyroidnodules between fine needle aspiration and core needle biopsy.Methods:179patients with thyroid nodules were performed notonly fine needle aspiration but also core needle biopsy, of which138cases who received surgical therapy were considered objects in this study.Compare the pathologic results of fine needle aspiration and core needlebiopsy to postoperative pathologic results referred as the diagnosisstandard. compare the results of two methods with X~2test and comparethe accuracy, sensitivity, specificity, positive predictive value, negativepredictive value, false positive rate, false negative rate of the two methodswith X~2test.Results: the accuracy, sensitivity, specificity, positive predictivevalue, negative predictive value, false positive rate, false negative rate offine needle aspiration were87.7%(121/138),87.1%(81/93),88.9%(40/45),96.4%(81/84),76.9%(40/52),6.7%(3/45),12.9%(12/93), respectively;the accuracy, sensitivity, specificity, positive predictive value, negativepredictive value, false positive rate, false negative rate of Core needlebiopsy were92.8%(128/138),95.7%(89/93),86.7%(39/45),93.7%(89/95),90.7%(39/43),13.3%(6/45),4.3%(4/93), respectively. Althoughthere was no significant difference between fine needle aspiration andcore needle biopsy as a whole(P=0.118>0.05), sensitivity, negativepredictive value and false negative rate were significantly different (P=0.039,0.007,0.022, respectively).Conclusion:(1) There was no significant difference in specificity between fineneedle aspiration and core needle biopsy.(2) Core needle biopsy was significantly better than fine needleaspiration in diagnosis of malignant thyroid nodules.(3) Fine needle aspiration was more applicable than Core needlebiopsy when the nodule was small, next to common carotid artery,hypervascular, next to the superior or inferior portion of thyroid or atdeep zoon.(4) It is necessary to combine with core needle biopsy when fineneedle aspiration only can not receive definite diagnosis.
