Clinical Investigation On Integrative Medical Treatment Of Cesarean Scar Pregnancy

PURPOSE:This paper is to study the treatment of traditional Chinese medicalempirical formula combined with methotrexate on cesarean scarpregnancy (CSP), the comprehensive evaluation of the role ofcombining traditional Chinese and western medicine therapy in thetreatment of CSP, aimed to find a new conservative treatment.METHOD:43cases of CSP patients were chose from inpatient department ofWuhan Integrative TCM﹠western Medicine Hospital among02/2011to02/2013, all of the cases were diagnosed with cesarean scarpregnancy,but unsolved,the vital signs are stable.None of the patientshad contraindication in using herb and MTX.All of them choseconservative medical treatment.The patients were divided into2groups randomly: one is westernmedicine treatment group(15cases), the other one is traditionalChinese medicine (TCM) and western integrative treatment group(28cases). Western medicine treatment group were treated with localinjection of MTX(50mg/m2) guided by ultrasound to kill embryos; TCMand western integrative treatment group Group were treated with local injections of MTX50mg/m2, at the same time drunk the empiricalformula called ectopic pregnancy II drink in7days for promoting bloodcirculation and cutting down HCG.In the process of treatment,we should survey hemanalysis、hepaticfunction、HCG and ultrasound;record adverse reactions of the twogroups.After the treatment, all the patients were followed-up for twomonth,keeping on survey of HCG until it dropped to normal, andrecording the time when vaginal bleeding stop and menstruationrecovery.We use SPSS16.0software for statistical analysis. Two-sides test wasadopt in our statistically significant inspection with P <0.05as thecriterion by judging the significant differences.RESULT:1、In the aspect of clinical efficacy: After treatment, the single treatmenteffective rate was40.0%for control group,its total effective rate was73.3%. The single treatment group effective rate was78.6%fortreatment group,its total effective rate was92.9%. The different of thetwo treatments is significantly (P <0.05). Treatment group’s effectiverate was higher than Control group.2、The time of HCG reduced to under100IU/L in treatment group wasless than that in control group. At the same time, the total hospitalizationtime、vaginal bleeding time、menstrual recovery time in treatment groupwere less than that in control group.3、In terms of adverse reaction,like vomiting、nausea、liver dysfunctionand white blood cell decrease,the diffrence between two groups had nostatistically significant(P>0.05). CONSULT:Traditional Chinese medicine (TCM) combined local injection ofMTX in treatment of pregnancy on uterine scar after cesarean section issuperior to semplice local injection of MTX. With its uniqueness, theclinical curative effect of Traditional Chinese medicine in CSP medicineconservative treatment is obvious.It can induce a fast decline of HCG,with the advantages of less time in hospitaland fewer side effects.Traditional Chinese medicine (TCM) combined with methotrexate intreatment of cesarean scar pregnancy conservative treatment canimprove the medicine conservative treatment effective rate, cut downthe time in hospitaland, and reduce adverse reaction.It can let patientsavoid from the surgical trauma, retained the reproductive function inmaximum.By less side effect, lower medical costs at the same time, itreduce the anxiety of patients and their families.
