Study On Standardized Training Of Specialized Nursing Operations For Junior Nurses

Junior nurses are joining the clinical nursing with an increasingly high proportion,it brings power to clinical work, but at the same time, it also buries hidden dangers forclinical nursing. There is no standardized training program with training content andway and time in Beijing. And lead to an uneven training effect. Especially thespecialized nursing operation is not standardized. Thus, it is extremely important toconduct standardized trainings of specialized nursing operation for junior nurses.Objective: Make sure the training content and connotation for the junior nurses indifferent hierarchy of specialized nursing operation. Collection the specialized nursingoperations and classification which are used by orthopedics, general surgery and soon.Make sure that the nurses of different departments are responsible for specializednursing operation training items, and determine the degree of difficulty and thedifficulty classification of every training item, in order to formulate the training outlineon the standardized training of the specialized nursing operation for junior nurses. Itenables the nurses to quickly master the frequently-used specialized nursing operationesand the specialized nursing operation training is standardized.Methods: Through the Delphi method, this study adopts the self-designed ExpertConsultation Questionnaire on the Standardization Training for Junior Nurses ofSpecialized Skills” and21nursing experts in Beijing are chosen for the expertconsultation. The dates of the valid questionnaires are censured and the categories,content and connotation of specialized nursing skills training items for the nurses indifferent hierarchy are analyzed. By using methods like interviewing, literature reviewand so forth, we could understand the items of commonly used nursing operation, whichare represented by orthopedics, general surgery department, thoracic surgery department,cardiology department, pneumology department and digestive department. They wereall classified, and the Expert Consultation Questionnaire on Standardization Trainingfor Junior Nurses of Specialized Operation” was designed on this basis, selecting17nursing experts for consultation in each research department. It carries on the statisticsof the effective questionnaires, and analyzes the training items of specialized nursing operation, the degree of difficulty and the categories of these items.Results:1. The training on the nurses in different hierarchy specialized nursing skills for juniornurses can be divided into five categories, including the specialized nursing theory,specialized nursing operations, nursing education, and nursing research and nursingadministration, and the specialized nursing operations trainning occupies animportant proportion.2. We have divided the specialized nursing operations into four categories: specializednursing operations in common use, pipeline care technology, doctor nursecooperation, specimen collection and index interpretation techniques.3. We have determined the trainings of six departments including orthopedics, generalsurgery department and so on for junior nurses in different hierarchy and the levelsthey should be mastered.4. We have determined the degree of difficulties of each operating technology incommon use, and classified specialized nursing operations into three categories dueto the degree of difficulty: the techniques that are less difficult, have safe clinicalapplication and are approved by the efficacy are the first-class techniques. Theoperation techniques which have a certain degree of difficulty, risk andinvasiveness are called the second-class techniques; those operation techniqueswhich have a high level of difficulty and risk, the new techniques applied in theearly stage of clinic, and those whose safety and effectiveness still need verificationare called the third-class techniques.Conclusions:1. Draw up different training outlines according to the different training contents forjunior nurses in different hierarchy and features of departments, so as to conductspecific trainings accordingly.2. Nursing manager can divide according to the degree of difficulties specializednursing operations and make plans about trainings accordingly and match differentsources of teachers.
