Background and Objective Pregnancy is a natural phenomenon of homologous transplantation, the maintenance of normal pregnancy is a process of organism inflammatory coordination reaction. Studies suggest that the majority of Thl type cytokines of early normal pregnancy decides immune elimination, in late pregnancy may be related to the relative immunosuppression of Th2type cytokines.Enter the deliver, large amounts of inflammatory cytokines into the uterus, promote recurrent inflammation, causing uterine launch the expulsion of the fetus. Thl/Th2cytokine immune imbalance can lead to the occurrence of a lot of pregnancy complications such as preeclampsia, abortion, premature delivery. Preeclampsia is a pregnancy-specific disorder, which pathogenesis is associated with a variety of factors, and the vascular endothelial injury is the center link. Recent studies indicate that preeclampsia is an excessive inflammatory reaction of pregnancy, vascular endothelial injury is part of intravascular excessive inflammatory response, which involves white blood cells, blood coagulation and complement system and other factors, chemerin is a novel adipokine associated with immune inflammatory reaction. When the inflammation, injury, bleeding occurred, it can make the immaturity plasmacytoid dendritic cells (pDC) of chemR23across the vascular endothelial cells, and arrive at the site of inflammation to elicit an immune response. The current research on chemerin in pregnancy is limited in gestational diabetes mellitus, and information about relationship between chemerin and normal pregnancy and preeclampsia is less. Through the study of chemerin expression level in different periods of normal pregnancy serum and placenta tissue, and in the third trimester and preeclampsia serum and placental tissues, in order to explore the role of chemerin in normal pregnancy and preeclampsia development, providing clues for further exploring the pathogenesis of preeclampsia.Methods A total of25preeclampsia patients and65normal pregnancies (women who did artificial abortion (20cases), induction of labor (15cases), parturient underwent cesarean section at term pregnancy (30cases), preeclampsia pregnant women underwent cesarean section (25cases)) who underwent regular antenatal examinations in Qilu hospital, Shandong University. Blood from the median cubital vein was drawn before treatment, and were labeled. Placental tissues were obtained immediately after the delivery, chemerin were detected by enzyme-linked immunosorbent assay (ELISA)in different periods of normal pregnancy serum and compare the level of chemerin between normal late-pregnancy and preeclampsia, chemerin in placental tissue was detected by immunohistochemical SP method and RT-PCR technique.Result (1) Compared to the first trimester pregnancy,the expression of chemerin was evidently higher in threatened labor group (382.09±54.26ng/ml vs273.51±39.54ng/ml)(P<0.05);the expression of chemerin in the second trimester group has increased,but the difference was not significant (337.88±70.65ng/ml vs273.51±39.54ng/ml)(P>0.05)(2) chemerin protein was mainly expressed in the syncytiotrophoblast,a little expression in trophoblast cells, and the level of chemerin increased with the gestational age growth. Compared to the first trimester pregnancy, the IOD of chemerin protein in placenta was significantly increased in late-pregnancy group(0.09558±0.0417vs0.03539±0.0285)(P<0.05). Compared to the first trimester pregnancy, the mRNA of chemerin in placenta was significantly increased in late-pregnancy group (P<0.05); the mRNA of chemerin in the second trimester group has increased,but the difference was not significant (P>0.05).(3) Fetal birth weight in patients with preeclampsia was significantly lower than that of the same gestational normal late pregnancy group (P<0.05);compared to the normal late-pregnant group,the expression of chemerin in serum was increased significantly in preeclampsia patients (456.98±77.71ng/ml vs382.09±54.26ng/ml)(P <0.05)Conclusion The occurrence and development of normal pregnancy may depend on the low level of chemerin, the local immune inflammatory response caused by high levels of chemerin may lead to the fetal delivery. The levels of chemerin in serum and placenta elevated in preeclampsia patients may associate with the proinflammatory effect of chemerin, and may be involved in the occurrence and development of preeclampsia.
