Objective: Pneumonia is a common disease and frequently encountereddisease of respiratory tract infection.Especially children and the elderly aremore likely to develop respiratory disease. Traditional Chinese Medicinethinks the lung controls Qi and breathing. Lung being the delicate organ, lungbeling the canopy of the human body. External environment changes candirectly affect the lung through respiratory tract physiological function andcause its function disorder, which leads lung disease. Western medicine thinksthat bacteria, viruses, and can cause lung inflammation. For bacterialpneumonia, the emergence of antibiotic used to make its case fatality ratesignificantly decreased, but in recent years, despite the use of powerfulantibiotics and effective vaccine, the mortality of pneumonia is continue togrow. The curative effect on western medicine treatment of viral respiratorytract infections is not good, especially in severe cases of respiratory infectionand virus proliferation, simple antiviral therapy can’t control it effectively.Traditional Chinese medicine in the treatment of pneumonia can also play agood role, such as Xuanbai Chengqi Decoction. Its efficacy is to clear awarylung-heat and relieve asthma, with the effect of pilling fever and purging. It iswidely used in the actual clinical disease in respiratory system, To tracheitis,bronchitis, acute pneumonia, it also has good curative effect, such as chronicobstructive pulmonary diseases and acute exacerbation period and corpulmonale with acute attack period and so on. Its curative effect has receivedthe affirmation of the broad masses of medical workers in the medicalcommunity. But just using traditional Chinese medicine especially intreatment of acute pneumonia is slower, so in order to get the best curativeeffect, most physicians adopt combine traditional Chinese and westernmedicine treatment. A large number of clinical observation also confirmed that adopting combination of traditional Chinese and western medicinetreatment is better than that of pure traditional Chinese medicine and westernmedicine antimicrobial therapy. However, the therapeutic effect of Chineseand western medicine treatment on contrast experiment and the mechanismresearch of Xuanbai Chengqi Decoction has not yet been reported. In order tofurther explore the best treatment of acute pneumonia and the mechanism ofaction on Xuanbai Chengqi Decoction, we do the experiment contrast researchabout Xuanbai Chengqi Decoction, cefotaxime sodium and Chinese medicinecombined with western medicine treatment of acute pneumonia(type ofphlegm heat resisting lung)in rats. Through monitoring the temperature,observating the stool form, quantity and the habits changes of the acutepneumonia rats, and measuring nitric oxide(NO), interleukin-6(IL-6),c-reactive protein(CRP)in blood of rats and the expression of lung tissueTNF-α changes of rats in the indicators, we explore the mechanism of actionabout XuanBaiCheng qi decoction in rats of acute pneumonia(type of phlegmheat resisting lung), through Xuanbai Chengqi decoction, cefotaxime sodiumand the combination of Chinese and western medicine(type of phlegm heatresisting lung)in treatment of acute pneumonia in rats with experimentalcontrast research, providing reliable theoretical and experimental basis for thetreatment of clinical acute pneumonia(type of phlegm heat resisting lung)and for clinical and teaching of traditional Chinese medicine services.Methods: Select40female Wistar rats of healthy and clean grade atweight of210~240g. In accordance with the principles of randomizeddivision, dividing them into5groups,8rats in each group: Normal ControlGroup (which was abbreviated to “N”group), Model group(which wasabbreviated to “M”group), Cefotaxime Sodium Group(which was abbreviatedto “C”group), Xuanbai Chengqi Decoction Group(which was abbreviated to“X”group), Chinese and Western medicine Group(which was abbreviated to“CW”group). The above groups were fed for one week, observing their livinghabit during every morning at8:00, measuring anal temperature,establishingthe model of acute pneumonia in rats. After one week, except the normal control group, other groups were instillated Escherichia coli by using thetracheal insertion method, and instillation dose is0.025ml. Then observe thechanges of basic situation of rats: mental status, respiratory rhythm, dyspnea,hair, aggregation, drinking and eating etc.After24hours of treatment, thenormal control group and model group was filled the stomach with0.9%normal saline, Cefotaxime sodium group intramuscular injection ofCefotaxime sodium injection, Xuanbai Chengqi Decoction group was filledthe stomach with Xuanbai Chengqi Decoction, Chinese and Western medicinegroup was filled the stomach with Xuanbai Chengqi Decoction andintramuscular injection of cefotaxime sodium for injection. During theadaptive feeding,and after established the model and during the treatment, allthe rats in each group were measured anal temperature at morning8:00, andeach rat was measured three times, using the average as the actual temperature.After the experiment, using10%chloral hydrate anesthesia, and then takingthe femoral artery blood, measuring the required indicators,and then quicklytaking out right pulmonary lower lobe of rats lung tissue, do the pathologicobservation by HE staining, under light microscopy to observe the basicmorphological changes. TNF-α was measured by immunohistochemicalmethod to detect the expression in the lung tissue; NO was measured bynitrate reductive enzymatic; IL-6and CRP were measured byradioimmunoassay.Results:1General observationDuring the building period,there was no significant difference betweenthe normal group and the adaptability of the feeding period, the rats eat less,but the amount of drinking water increased, dry stool black quantity is little,poor mental state, activity decreased, hair stands, shortness of breath andasthma with protrusions of the ear marginal vein protuberant. After treatment,compared with model group, cefotaxime sodium group, Xuanbai ChengqiDecoction group and chinese and western medicine group rats significantlyimproved mental state, activity increased,the hair gradually restore luster, reduction of water intake, shortness of breath relieved and ear marginal veinwithout protrusions. Cefotaxime sodium group rats are with dry stool,increasingparticles; stool surface of XuanBaiCheng qi decoction group andChinese and western medicine group changes to be wet, defecate increase butno diarrhea.2the influence of the temperature in ratsCompared with the normal control group, model group rats’ bodytemperature elevated(P <0.05); compared with the model group, cefotaximesodium group, Xuanbai Chengqi Decoction group, Chinese and westernmedicine group, the body temperature of rats were decreased(P <0.05); therewas no significant difference between treatment groups(p>0.05).3The measured results of each group in rats3.1The results of the number of WBC and PMN in the blood of ratsThe results of WBC, compared with the normal control group, thenumber of WBC in the model group rats increased significantly(P <0.05).Compared with model group, the number of WBC in cefotaxime sodiumgroup, Xuanbai Chengqi Decoction group, Chinese and Western medicinegroup were significantly decreased(P <0.05). Cefotaxime sodium group andXuanbai Chengqi Decoction group showed no significant difference(P>0.05),compared withcefotaxime sodium group and Xuanbai Chengqi Decoctiongroup, chinese and western medicine group was decreased significantly(P <0.05).The results of PMN, compared with the normal control group, thenumber of PMN in the model group rats increased significantly (P <0.05).Compared with model group, the number of PMN in cefotaxime sodium group,Xuanbai Chengqi Decoction group, Chinese and Western medicine groupwere significantly decreased(P <0.05). Cefotaxime sodium group andXuanbai Chengqi Decoction group showed no significant difference(P>0.05),compared with cefotaxime sodium group and Xuanbai Chengqi Decoctiongroup, chinese and western medicine group was decreased significantly(P <0.05). 3.2The results of the content of NO in the blood of ratsCompared with the normal control group, the content of NO in blood ofrats in the model group was significantly increased(P <0.05), compared withthe model group, the content of NO in cefotaxime sodium group, XuanbaiChengqi Decoction group and chinese and western medicine group were allobviously decreased(P <0.05), compared with the cefotaxime sodium groupand Xuanbai Chengqi Decoction group, chinese and western medicine groupwas decreased obviously, there was significant difference(P <0.05). Therewere differences amog the three groups(P <0.05): cefotaxime sodium groupwas lower than Xuanbai Chengqi Decoction group(P <0.05), chinese andwestern medicine group was lower than cefotaxime sodium group(P <0.05).3.3The results of the content of IL-6in the blood of ratsCompared with the normal control group, the content of IL-6in themodel group rats increased significantly(P <0.05). Compared with modelgroup, the content of IL-6in cefotaxime sodium group, Xuanbai ChengqiDecoction group, Chinese and Western medicine group were significantlydecreased(P <0.05). Cefotaxime sodium group and Xuanbai ChengqiDecoction group showed no significant difference (P>0.05), compared withcefotaxime sodium group and Xuanbai Chengqi Decoction group, chinese andwestern medicine group was decreased significantly(P <0.05).3.4The results of the content of CRP in the blood of ratsCompared with the normal control group, the content of CRP in themodel group rats increased significantly(P <0.05). Compared with modelgroup, the content of CRP in cefotaxime sodium group, Xuanbai ChengqiDecoction group, Chinese and Western medicine group were significantlydecreased(P <0.05). Cefotaxime sodium group and Xuanbai ChengqiDecoction group showed no significant difference(P>0.05), compared withcefotaxime sodium group and Xuanbai Chengqi Decoction group, chinese andwestern medicine group was decreased significantly(P <0.05).4pathology morphological observation of lung tissue under light microscopeLung tissue pathology showed: normal control group rats alveolar interval has a few inflammatory cells and alveolar cavity is clean, no edemafluid, no hemorrhage and no inflammatory cells infiltration. Model group rats’alveolar interval was widened significantly, there is a large number ofinflammatory cells infiltration, liquid in alveolar edema, and hemorrhage.Compared with the model group, cefotaxime sodium group and XuanbaiChengqi Decoction group alveolar interval narrowing, there was no liquid inalveolar edema, no hemorrhage and no inflammatory cell infiltration.Compared with cefotaxime sodium group and XuanBai Chengqi Decoctiongroup, Chinese and western medicine group rats alveolar interval narrowing,infiltration of inflammatory cells decreased significantly, the alveolar cavitywithout edema fluid, no hemorrhage and no inflammatory cell infiltration.5Expression of TNF-α in lung tissue by immunohistochemistryBrown area is positive expression area. Compared with the normalcontrol group, the expression of TNF-α in rats of model group increasedsignificantly(P <0.05); compared with the model group, the expression ofTNF-α in cefotaxime sodium group, Xuanbai Chengqi Decoction group andchinese and western medicine group were all obviously decreased(P <0.05);cefotaxime sodium group was lower than Xuanbai Chengqi Decoction group(P <0.05); chinese and western medicine group was lower than cefotaximesodium group (P <0.05).Conclusions:1Xuanbai Chengqi Decoction and cefotaxime sodium can all be used foracute pneumonia (type of phlegm heat resisting lung) treatment in rats. But thecombined medication of Xuanbai Chengqi Decoction and cefotaxime sodiumis better than pure chinese medicine of Xuanbai Chengqi Decoction orWestern medicine treatment of cefotaxime sodium. It can significantlyimprove the acute pneumonia (type of phlegm heat resisting lung) treatmenteffects in rats. So the method of Xuanbai Chengqi Decoction combined withcefotaxime sodium can be used as the first choice in clinical treatment of acutepneumonia.2The mechanism of action of clearing awary lung-heat and relieving asthma with the effect of pilling fever and purging of Xuanbai ChengqiDecoction can effectively reduce the number of WBC and PMN in the bloodof rats, decrease the content of NO, IL-6and CRP in the blood of rats, lowerthe expression of TNF-α in lung tissue of rat and also can make alveolarinterval narrowed in the rats of acute pneumonia(type of phlegm heat resistinglung), no liquid in alveolar edema, no hemorrhage and no infiltration ofinflammatory cells,thereby reducing the lung infection and the damage to thelungs. It indicates the function of Description Xuanbai Chengqi Decoction toclear awary lung-heat and relieve asthma, with the effect of pilling fever andpurging by hypothermia, antiasthma, antibacterial, anti-inflammatory,promoting defecation and reducing the absorption of toxins.3It proves that the theoretical understanding of Xuanbai ChengqiDecoction with the function of clearing awary lung-heat and relieving asthma,with the effect of pill fever and purge is correct and can be used in clinicaltreament.
