Objective: To observe the expression change of Cartilage oligomeric matrixprotein (Cartilage oligomeric matrix protein, COMP) in normal lumbarintervertebral disc in the anulus fibrosus and degenerative lumbarintervertebral disc anulus fibrosus, and to explore the relationship betweenCOMP and degenerative lumbar intervertebral disc, and provide theory for earlyprevention and treatment of the lumbar disc degeneration.Methods:60patients of lumbar intervertebral disc were collected fromSeptember2010to September2012lumbar specimens from, divided into2groups.Degeneration group: a total of41cases, collected from Dali medical schoolaffiliated hospital orthopedic, diagnosed as the lumbar4-5intervertebralcanal stenosis or waist5S1intervertebral canal stenosis diseases.30casesof men and11cases of women, age range from36to65, the average age is51.12±7.49. Before the operation these patients were performed lumbar X-ray, CTand MRI as routine inspection. These degeneration group patients under generalcompound vein anesthesia on the posterior spinal canal decompression andintervertebral disc excision, intervertebral bone graft fusion and internalfixation (posterior lumbar interbody fusion, PLIF). Normal group: a total of19cases,collected from Dali medical school affiliated hospital orthopedic,diagnosis as the lumbar spine burst fracture (fracture segment from waist1to waist4).12cases of male,7cases of female, age range from25to58,theaverage age of47.15±10.13, before the operation these patients were performed lumbar X-ray, CT and MRI as routine examination, under generalcompound vein anesthesia on the anterior spinal canal decompression andintervertebral disc resection, titanium cage bone graft fusion and internalfixation. Take out all the anulus fibrosus issue from the lumbarintervertebral disc specimens, conventional paraffin embedding, making anulusfibrosus wax block, make5micrometer thickness of slice, part of slices afterHE staining, use light microscope observe degeneration group and normal groupanulus fibrosus of the pathological differences; Another part of the slicesafter40degrees baking for the night, with a pH of6.0,0.01mol/l citratebuffer antigen of heating line repair, according to the kit operation procedureline of normal and degenerative intervertebral disc annulus fibrosusorganization COMP immunohisto chemical staining, observe the exchange ofCOMP in anulus fibrosus issues, and the mean optical density value wascalculated by the image analysis software, the results with the mean+/-standard deviation, according to statistical analysis using SPSS13.0softwareto process data, according to the features of data, the Independent-samplesT test is used to analyze the data, all p <0.05, the results have statisticalsignificance.Results: HE staining under light microscopy is observed:In the normal group: the annulus fibrosus tissue arranged orderly.Degeneration group: the annulus fibrosus tissue arranged disorder, there ismyxoid degeneration change in the degenerate annulus fibrosus tissue.Immunohisto chemical method test of COMP in anulus fibrosus, the results areas follows:Immunohisto chemical stain is visible in the normal anulus fibrosus cellcytoplasm in yellow-tan granule; Visible increased obviously in thedegeneration of anulus fibrosus cell cytoplasm in yellow–tan granule. COMPexpression in normal group the average optical density value is0.21±0.20;Degeneration group COMP express the average optical density value is0.63±0.18; There have obvious difference (P <0.05). Conclusion:1. Cartilage oligomeric matrix protein in normal lumbar annulus anddegeneration of lumbar annulus are expressed, And degeneration of lumbarannulus are expressed higher than in normal lumbar annulus.2. Cartilage oligomeric matrix protein expressed in anulus fibrosusenhancement may be one of the protect reasons of intervertebral discdegeneration.
