| Objective To evaluate the clinical effects of six-character formula respiratory gymnastics on pulmonary function,endurance exercise,dyspnea,Body-Mass Index(BMI),quality of life (QOL) in patients with moderate to severe chronic obstructive pulmonary disease(COPD), compare the effects of the six-character formula respiratory gymnastics with the whole body respiratory gymnastics which is usually used in clinic.To seek an efficaciousã€cheapã€easy rehabilitation method applied to the community and home rehabilitation training for patients with COPD.Method Sixty-five patients on stable phase collected from First Affiliated Hospital of Fujian Medical University,Fujian Province People’s Hospital and the Second People’s Hospital of Fujian Province were recruited in this clinical trail.Five patients were exited the experiment(experimental group lost three samples;control group lost two samples ).At last sixty enrolled patients were randomly divided into experimental group(thirty-one samples) and control group(twenty-nine samples).The patients in both groups were given routine treatment and health education,while those in the experimental group were given six-character formula respiratory gymnastics training and those in the control group were given whole body respiratory gymnastics training.All patients should take training for three months,thirty minutes per time every day.All patients should fill the training time and feelings in the form after exercise.During the experiment,patients will receive regular follow-up through telephone or home visit by trainers.The index of forced expiratory volume in one second(FEV1),FEV1%pred,forced expiratory volume in one second / forced vital capacity(FEV1/FVC),exercise tolerance(six-minute walk distance(6MWD) ),MRC dyspnea score,Body Mass Index(BMI),St George’s Respiratory Questionnaire(SGRQ) scores were evaluated before and after respiratory training.Result1.Pulmonary functionIt was revealed by pulmonary function test that significant difference was only found in FEV1(P<0.01)in experimental group compared with pre-rehabilitation,while there were no significant differences in FEV1%and FEV1/FVC(P>0.05).But FEV1,FEV1%,FEV1/FVC of patients in control group has no significant difference(P>0.05).There were no significant differences between two groups(P>0.05).. 2.Exercise toleranceThere was significant increase in 6MWD after three months training in experimental group (P<0.01),and also has the significant difference in control group(P<0.01).The 6MWD after the training of six-character formula respiratory gymnastics was significantly longer compared with whole body respiratory gymnastics[(49.94±12.79)m vs.(34.97±23.87)m] (P<0.05).3.Symptom of dyspneaAfter 3 months training,there were significant decreases in MRC dyspnea score in both groups(P<0.01).And there was significant difference in decrease ranges between experimental group and control group[(1.38±0.66) scores v s.(0.86±0.51) scores](P<0.05).4.BMIAfter 3 months training,the BMI of experimental group has no significant increase (P>0.05),and also no difference in control group(P>0.05).There were no significant difference between two groups(P>0.05).5.BODE index scoreAfter 3 months training,there were significant decreases in BODE index score in both groups(P<0.01),while there were no significant differences between two groups(P>0.05).6.Quality of life(SGRQ scores)After 3 months training,the mean scores of SGRQ in experimental group:respiratory symptoms scoreã€limited activities scoreã€disease impacts scoreã€total score are all decreased significantly(P<0.01);and the scores of SGRQ in control group:respiratory symptoms scoreã€limited activities scoreã€disease impacts scoreã€total score are all decreased significantly(P<0.01).The scores of limited activities and disease impacts were decreased to a significantly larger extent in experimental group[(limited activities score 16.74±12.09: 11.71±5.92 P<0.05);(disease impacts score 13.98±7.53;10.27±6.00;P<0.05);(total score 14.09±6.72;10.19±4.25:P<0.05)],while no significant difference in score of respiratory symptoms between two groups(P>0.05).Conclusion Six-character formula respiratory gymnastics does good to stable COPD patients,they could delay the progressive decline of pulmonary function,relieve symptoms of dyspnea,improve exercise tolerance and the quality of life.And six-character formula respiratory gymnastics has more advantages in relieving symptoms of dyspnea,improving exercise endurance and the quality of life when compared with the whole body respiratory gymnastics.So six-character formula respiratory gymnastics could be used as a more effective method for community and home rehabilitation on COPD. |
