Object ive:We compared MR I diagnosis with clinical diagnosis of anterior cruciate ligament injury by various analysis and discussion which were mainly based on scanning direction of MRI and reading MRI by doctors also based on the anatomical structure of anterior cruciate ligament,MRI imaging principle and MRI imaging diagnostic reference information when anterior cruciate ligament injuried to improve clinical diagnostic accuracy.Methods:From March in2011to January in2012, there are48cases suspected anterior cruciate ligament injury through clinical diagnosis compared with subsequent MRI and arthroscopy examination. MRI we used with1.5T fields was produced by GE company in the US.The results of MRI examination were diagnosed by an experienced radiological doctor. Related orthopedic physician made the final results after discussion and the arthroscopic examination made by the related orthopedic physician. To the results of arthroscopy as a standard to compare the result of differences between the two. Analyzed and classified the cases in order to supplement the insufficient number of cases. Try to analysis reference information showed by MRI imaging diagnosis in the way of the anatomical structure of anterior cruciate ligament and MRI imaging principle.We identify the relevant factors by put focus on analysising the cases of differences.Result:In our study,48anterior cruciate ligaments were diagnosed with the result that17normal anterior cruciate ligaments and31abnormal anterior cruciate ligaments in MRI diagnosis,To the results ofarthroscopy as the standard,MRI diagnostic results:27true-positive,13true-negative,4false-positive and4false-negative. Combined the relevant literature to a number of discussions we summarized the factors affecting MRI imaging diagnosis are as follows:(1)Scanning direction of MRI:1.Scanning direction and the anterior cruciate ligament is not the direction parallel which leads to low-rise section imaging and affects the judgment.2. Surrounding tissue hematoma in patients with acute lead to a false impress ion.3.Partial volume effects lead to a false impression. (2) Reading MRI by doctors:1. Unable to identify some details of the incomplete tear.2. Unable to identify patients with abnormal morphology of the anterior cruciate ligament images, including chronic degenerative conditions, and born with deformities.Conclusion:MRI as the first choice of imaging diagnosis, there is a very large clinical diagnosis of guiding significance and MRI is a better diagnosis of anterior cruciate ligament injury, examination methods. But under certain conditions it will be constrained by other factors, thus demonstrating the uncertainty.For these special circumstances, clinicians should have a sufficient understanding in order to improve the diagnostic accuracy.Cl inicians should pay attention to the following aspects to improve the diagnostic accuracy of MRI reading:1. Distinguish between patients with stage and to distinguish between patients with stage, pay attention to the possibility of false positive acute phase and the stale of the possibility of false negative.2. At tent ion to the patient’s age, pay attention to rule out the possibility of patients with chronic degenerative and congenital malformations.3.Pay attention to the quality of imaging on the MRI images to determine its credibility.
