Objective:Aim to analysis efficacy for the Pulmonary metastatic tumor treatment after lobectomy or subsequent and evaluate feasibility of thoracoscopic lobectomy for Pulmonary metastatic tumor and prospect its development.Methods:From January2005to January2007,45cases of patients (30men,15women, age range35to75years) underwent completely thoracoscopic lobectomy for metastatic carcinoma identified by pathological examination. the operation time, intraoperative bleeding, lymph node dissection, analgesics, drainage, postoperative hospital stay, postoperative complications,5years survival rate of patients were observed and evaluated with affected factor analysis.Results:1. Operation group45cases are successful, operation resection rate was100%, no peri operation period of death. Operation time (145.6+51.9); intraoperative bleeding volume (191.4ml+69.3); lymph node dissection (12+4.2); postoperative Taylor Ning doses (6+2.2); postoperative thoracic closed drainage time (4.1+0.6), the first3days of the pleural drainage (682.3+399.4) ml the mean postoperative hospital stay,(9.1+2)D.2. postoperative complications This group of45patients with postoperative complications occurred in3cases, the complication rate was6.67%, respectively for postoperative hemorrhage in1cases,1cases of severe hypoxemia, pulmonary air leak in1cases, after the symptomatic treatment, to become better.3. Patients with postoperative survivalThis group of45patients, follow-up time of4-60months, the average survival time for the (20.2+10.8) months,1year survival rate of72.4%,3annual interest rate37.97%,5year survival rate was31.03%.4. Survival after resection in patients with analysis of influencing factors In45cases of patients with postoperative survival rate influence factor analysis, chi-square analysis of visible, and gender, age, primary cancer staging, staging of the lesion, intraoperative bleeding are not the survival factors of Pulmonary metastatic tumor patients after resection. multivariate conditional regression analysis showed, gradually lgoistic diamond DFI is affecting Pulmonary metastatic tumor carcinoma prognosis independent factor.Conclusion:thoracoscopic lobectomy in diagnosis and treatment of the dual role, the use of safe and effective, less trauma, fewer complications, high survival rate, for a variety of Pulmonary metastatic tumor with operation indications, should give the thoracoscopic lobectomy.
