PurposeAnkylosing Spondylitis (AS) is a chronic, inflammatory and autoimmune disease which mainly affects the axial skeleton with variable involvement of peripheral joints, ligaments and tendons. No conventional treatment has proven to be effective in this disease. The prognosis of the disease usually involves sacroiliac joint, spine, soft tissue that surrounds spine and peripheral joints. Approximately45%of AS patients start from the peripheral joints. For an instance, hip joints. This is also a major reason to cause disability in AS. This trail aims to explore and discuss the effectiveness of Bu Shen Shu Ji Ke Li in treating AS with hip joint involvement, providing a solid clinical research and scientific evidence for the future trail.MethodThis clinical research was a randomized controlled trail. Sixty patients (60) with defined as Ankylosing Spondylitis and hip joints involvement were randomized into either Bu Shen Shu Ji Ke Li group (treatment group) or Sulfasalazine group (control group). After6months of observation and treatments, the participants were assessed by the clinical signs and symptoms, BAS-G, BASFI. BASDAI scale, night pain, spinal pain, physician assessment, Traditional Chinese Medicine syndrome score, Harris scale, BASRI-hips scale, ESR and CRP. The safety index was also monitored. The outcomes were assessed by the ASAS20, curative effect evaluation of Traditional Chinese Medicine, Harris scale and BASRI-hips scales.ResultThe results have indicated the reduction of ESR and CRP level in all sixty participants. For the Bath AS patients, there were improvement in functional index, BASDAI, BASFI, night pain, spinal pain. Traditional Chinese Medicine syndrome score, occipital to wall distance, finger floor distance, range of movement in thoracic region, Schober test. Mandible to occipital to wall distance, sternum distance, Traditional Chinese Medicine syndrome score was better in the treatment group. After six months of treatments, there were23(76.67%) of ASAS20patients in control group,27(90%) in treatment group, significant statically difference (P<0.05); overall therapeutic effect was83.3%in control group,96.7%in treatment group, significant statically difference (P<0.05);90%of Harris scale improve in control group,96.7%in treatment group. There is no significant statically difference (P>0.05). It shows that Bu Shen Shu Ji Ke Li has therapeutic effect upon the AS with hip joint involvements in terms of reducing joint pain, and increasing function and range of movement. In addition, the adverse effect was no reported for the chronic use of Bu Shen Shu Ji Ke Li.ConclusionHip joint is the most commonly affected joint in Ankylosing Spondylitis, it is also the major reason to cause disability in AS. The Bn Shen Shu Ji Ke Li lias high therapeutic effect on the spine, range of movement for joint, improvement in signs and symptoms. Especially for the hip joint pain, function, lower limb deformation and range of movement. This trail has proven that Bu Shen Shu Ji Ke Li provides a better clinical therapeutic effect with no adverse effects. It also gives a scientific foundation for the further clinical research in the treatment in AS.
