The Study Of Computer Navigation And Blood Loss Management In Total Knee Arthroplasty Postoperative Blood Loss And The Incidence Of Venous Thromboembolism

ObjectiveCompare computer navigation technology and intra-articular cocktail recipescombined with occlusion of the drainage tube, the continuous ice compress for total kneearthroplasty in total blood loss, hidden blood loss, drainage and transfusion rates.Evaluation of computer navigation and blood loss management measures for venousthromboembolism morbidity.MethodsThis is a study of a prospective randomized study. Based on inclusion and exclus ioncriteria, select78patients with osteoarthritis implementation TKA from March2011toJune2012of The Fourth Military Medical University of XiJing Hospital. Underrandomization,25were assigned into the computer navigation and blood loss managementgroup(Group A),27were assigned into blood loss management group (Group B),26wereassigned into control group(Group C). Oral rivaroxaban (10mg/day) for VTE wasadministered for TKA patients from day1to14on postoperatively. Record the patient’spostoperative drainage and hematocrit of1day before surgery and on postoperative day5.Postoperative total blood loss, hidden blood loss is calculated according to the formula.Observed the occurrence of VTE in patients and in7days after lower limb venous Doppler ultrasound to record any part of the DVT, non-fatal PE and deathsResultsAccording to the amount of blood loss formula, the Group A postoperative blood loss,hidden blood loss, drainage volume were of1313.2±383.5ml,593.7±185.3ml,409.5±132.6ml.The Group B were1489.6±418.5ml,688.0±195.2ml,468.3±165.7ml.The Group Cwere1726.3±465.1ml,813.4±221.5ml,523.1±157.4ml. The DVT rate was12.0%(3/25) incomputer navigation group, and14.8%(4/27) in Group B, and11.5%(3/26) in the controlgroup.ConclusionComputer navigation technology and intra-articular cocktail recipes analgesia, andpostoperative occlusion of the drainage tube, the continuous ice compress postoperative ofmanagement measures can effectively reduce the total blood loss, hidden blood loss,drainage and allogeneic blood transfusion after TKA. These methods will not increase theincidence of postoperative VTE.
