Prognostic Factors For Survival In Patients With Advanced Metastatic Androgen-independent Prostate Cancer

Objective: To analyze retrospectively the information of survivalsituation about advanced metastatic androgen-independent prostatecancer(AIPC) in order to find the prognostic factors.Methods: Data from132advanced metastatic AIPC patiences with completemedical information between June2005and December2012,who were treatedin the First Affiliated Hospital of Fujian Medical University,wereretrospectively analyzed.Specific information was abstracted,thenunivariate and multivariate Cox regression,log-rank chi-square test andKaplan-Meier curves were computed using SPSS version17.0.univariate andmultivariate Cox regression were used to analyze the relationship betweenseveral variates and total survival rates of AIPC.these variates wereage,Gleason score,baseline PSA,hemoglobin(HB),alkalinephosphatase(ALP),lactate dehydrogenase(LDH),nadir PSA level afterhormone therapy and PSADT in AIPC.Results:（1）At the final follow-up,20cases were alive,102cases were dead and10cases were lost to follow up.Median survival from diagnosis of AIPCwas25±3.1months,and the one-year,two-year,three-year survival ratewas85％,53％,23%.（2）By univariate Cox regression analysis baseline PSA (HR=2.378，P＜0.001),HB（HR=2.245,P＜0.001）,ALP(HR=2.263，P=0.002)，LDH(HR=1.865，P=0.004),nadir PSA level after hormone therapy(HR=2.293，P＜0.001),PSADTin AIPC(HR=2.111，P＜0.001)were calculated to be independent prognosticfactors of advanced metastatic AIPC.Age and Gleason scores have nothingto do with the survival of advanced metastatic AIPC.（3）By multivariate Cox regression analysis baseline PSA(HR=1.776， P=0.008),HB（HR=1.869,P=0.005）,ALP(HR=1.915，P=0.021)、LDH(HR=1.944，P=0.004),nadir PSA level after hormone therapy(HR=2.041，P=0.002),PSADTin AIPC(HR=2.026，P=0.001)were calculated to be independent prognosticfactors of advanced metastatic AIPC.Conclusion:baseline PSA,hemoglobin(HB),alkalinephosphatase(ALP),lactate dehydrogenase(LDH),nadir PSA level afterhormone therapy and PSADT in AIPC were independent prognostic factors ofadvanced metastatic AIPC.
