Background Tuberculous meningitis is a infectious disease which is worldwide and seriouely harmefulto human beings. The traditional normal assay of cerebrospinal fluid and radiological examination alwaysdelay the clinical treatment because of its low sensitivity and specificity. Therefore, we apply a moresensitivier method to the clinical diagnosis of the tuberculous meningitis.ObjectivesUsing T cell enzyme-linked immunospot assay to detect tuberculous meningitis peripheral blood andcerebrospinal fluid mononuclear cells, Compare the two methods diagnosis of tuberculous meningitissensitivity and specificity, For the early diagnosis of tuber Culous Meningitis and provide effective methodin this article.MethodsCollect respectively30cases of tuberculous meningitis patients and30cases of non-tuberculousmeningitis patients of peripheral blood and cerebrospinal fluid, separate and extract mononuclear cells,through cryopreservation and recovery, A spot test using T lymphocytes infected with TB (T-SPOT-TB)was evaluated for the sensitivity and specificity in the diagnosis of tuberculous meningitis. Results CSFmononuclear cells ELISPOT assayResultsThe case group were30cases, of which28cases were diagnosed as tuberculous meningitis and2cases were diagnosed as non-tuberculous meningitis, the positive rate was96.7%; Control group was30cases,1cases was diagnosed as tuberculous meningitis,29cases were diagnosed as non-tuberculousmeningitis, sensitivity:93.3%; specificity:96.7%, false Positive rate:3.3%; f alse negative rate:6.7%; Positive likelihood ratio:28.3; Negative likelihood ratio:0.07. Eripheral mononuclear cells ELISPOT assayResult The case group were30cases, of which23cases were diagnosed as tuberculous meningitis and7cases were diagnosed as non-tuberculous meningitis, sensitivity:76.7%; Control group was30cases,4cases was diagnosed as tuberculous meningitis,26cases were diagnosed as non-tuberculous meningitis,pecificity:86.7%; false Positive rate:13.3%; false negative rate:23.3%; Positive likelihood ratio:5.8;Negative likelihood ratio:0.15. Fisher’s Exact tests was used in the positive rate of uberculous meningitisperipheral blood ELISPOT assay results and cerebrospinal fluid ELISPOT assay results, Both results wereno statistical significant differences (P=0.254).ConclusionsPeripheral mononuclear cells ELISPOT assay and CSF mononuclear cells ELISPOT assay can be usedas the current effective methods of tuberculous meningitis. And CSF mononuclear cells ELISPOT assay isHigher sensitivity and specificity than peripheral mononuclear cells ELISPOT assay.
