Objective To investigate factors that may influence the clinical outcomesof acute ischemic stroke patients treated with intravenous recombinanttissue plasminogen activator.Methods This is a retrospective analysis. One hundred and one patients withacute ischemic stroke who received recombinant tissue plasminogen activatorintravenous thrombolysis within4.5hours from disease onset were included.Patients were divided into good or poor outcome group according modifiedRankin Scale score at90days (good outcome group: Rankin Scale score of0-1;poor outcome group: Rankin Scale score of2-6). Stroke characteristics werecompared between the two groups. The factors that affect stroke outcomes wereanalyzed via univariate analysis and logistic regression analysis.Results Of the101patients studied, patients in good outcome group (n=55)were significantly younger than patients in poor outcome group (n=46,P=0.029). Good outcome group had fewer patients with diabetic history(P=0.012) and presented with lower blood glucose level (P=0.012), lowersystolic blood pressure level (P=0.003), lower baseline NIHSS score (P=0.002)and shorter onset-to-treatment time (P=0.044) than poor outcome group.Logistic regression analysis showed,compare with poor outcome group, goodoutcome group showed lower baseline NIHSS score (OR0.885,95%CI0.793-0.989),lower blood glucose level (OR0.699,95%CI0.491-0.994), lower systolicblood pressure level (OR0.962,95%CI0.929-0.997) and not previous historyof diabetes(OR0.968,95%CI0.941-0.996).Conclusion Patients with low baseline NIHSS score, lower blood glucose level, lower systolic blood pressure and not previous history of diabetesbefore thrombolysis treatment have a better outcome. |
