OBJECTIVE:To study the change of peripheral blood T cell subsects and NK cell duringperioperation and its clinical significance in patients with gliomas.METHODS:From October2011to April2013,42cases of primary intracranial tumors with30gliomas and12benign tumors involved meningiomas, hypophysoma and so on. hospitalized wereinvestigated.Of30gliomas,17cases were high-gradegliomas (WHO,grade III-IV),and13cases werelow-grade gliomas (WHO,grade I~II).Of these42patients,male19cases,female23cases,midof age is43.8years (23-72).In addition,there were10healthy controls,with4male cases and6femalecases, whose average age is40.4.To draw2ml peripheral blood of all these patients in1day beforeoperation and24hours,1week,2week,3week and4week after operation.Then detect T lymphocytesubsets and NK cell by FACS.RESULTS:The statistical analysis results:①Compared with healthy controls, the level ofCD3+, CD4+and CD56+percentage and the CD4+/CD8+ratio was significantly decreased both inhigh-grade glioma and low-grade glioma,while CD8+was increased,these changes have statisticaldifference(P<0.01).②Compared with benign tumors, the level of CD3+, CD4+and CD56+percentage and the CD4+/CD8+ratio was significantly decreased range from1day before operation to4week after operation both in high-grade glioma and low-grade glioma, these changes also havestatistical difference(P<0.05);the level of CD8+percentage was significantly increased range from1day before operation to2week after operation in high-grade glioma,but decreased during the thirdand the fourth week after operation(P<0.05); the level of CD8+percentage was increased during1day before operation and24hours after operation in low-grade glioma(P<0.05).③Compared withlow-grade glioma, the level of CD3+and CD4+percentage was significantly decreased range from1day before operation to4week after operation both in high-grade glioma(P<0.05); the level of CD8+percentage was significantly increased range from1day before operation to2week after operation inhigh-grade glioma,but decreased during the third and the fourth week after operation(P<0.05); Thepercentage of CD56+was decreased range from1day before operation to3week after operation in high-grade glioma(P<0.05),but has no significant difference at the fourth week after operation; Theratio of CD4+/CD8+was decreased during1day before operation and24hours,1week and4weekafter operation in high-grade glioma(P<0.05), but has no significant difference at the second and thirdweek after operation.④Compared with1day before operation, the level of CD3+,CD4+percentageand the CD4+/CD8+ratio was significantly decreased at24hours after operation both in high-gradeglioma and low-grade glioma(P<0.01), while CD56+and CD8+was differently increased(P<0.05);The percentage of CD3+and CD4+and the ratio of CD4+/CD8+both in high-grade glioma andlow-grade glioma at24hours after operation had differently lower levels than those range from1weekto4week after operation(P<0.01),which was significantly lower in high-grade glioma than those innormal status(P<0.05); The percentage of CD8+at24hours after operation had a higher level thanthat range from1week to4week after operation in high-grade glioma(P<0.05),while those inlow-grade glioma had no significant difference; Compared with24hours after operation, the percentageof CD56+at1week after operation was decreased(P<0.01),but increased range from2week to4week after operation which had a lower level than that at24hours after operation(P<0.05).CONCLUSION: The cellular immune function in glioma patients was depressed, and was relatedwith glioma malignace. The suppression in the patients with malignant glioma is more serious than thepatients with benign tumors. Operative stress may cause a change of activity of T cell subpopulationand NK cell. The cell immune function of the tumorresectional patients presents a process fromtemporary suppression to gradual recovery.
