Objective To explore relationship between brachial-ankle pulse wave velocity(baPWV) and adiponectin level and gene polymorphism in promoter region inpatients with type2diabetes mellitus.Methods184patients with type2diabetes mellitus, including111patients withincrassation of baPWV in DM1group (baPWV>1400cm/s) and98patients withoutincrassation of baPWV in DM2group(baPWV≤1400cm/s) were enrolled in this study.Serum adiponectin level (APN), fasting plasma glucose (FPG), fasting insulin (FINS),glycosylated hemoglobin (HbA1c), total cholesterol (TC), triglyceride (TG), lowdensity lipoprotein (LDL-C), high density lipoproteins (HDL-C), body height, weight,waist circumference (WC), hip circumference (HC), systolic blood pressure (SBP),diastolic blood pressure (DBP), carotid artery intima media thickness (CIMT),brachial-ankle pulse wave velocity (baPWV) were measured. Body mass index (BMI),waist to hip ratio (WHR), homeostasis model assessment of insulin resistance(HOMA-IR) were calculated. The adiponectin gene SNP-11377Câ†'G polymorphismwas identified by polymerase chain reaction restriction fragment lengthpolymorphism (PCR-RFLP). The Hardy-Weinberg equilibrium was tested byChi-square test of goodness of fit, and polymorphic frequencies were compared inDM1and DM2groups. T test, Chi-square test, or rank-sum test was used to comparedifferences between groups. The association of adiponectin and other factors was testby stepwise multiple regression analysis.Results (1) The frequencies of adiponectin gene genotype and allele showedsignificant difference between DM1and DM2group (P<0.05).(2) Serum adiponectinlevel (mg/L) and HDL-C in subjects with non-SNP-11377CC group was lower than that in subjects with SNP-11377CC group, otherwise, baPWV, CIMT, FINS, and age washigher in non-SNP-11377CC group than that in SNP-11377CC group (P<0.05).(3)DM2group has much higher levels of baPWV, age, history of diabetes, SBP, TC,CIMT, but lower adiponectin level (P<0.05).(4) Simple correlation analysis provedthat plasma adiponectin level was negative correlated with baPWV, CIMT, HOMA-IR,LDL-C, history of diabetes, and age. Stepwise multiple regression analysis provedthat plasma adiponectin level was negative correlated with age, HbA1c, LDL-C, butpositive correlated with TC.Conclusion In T2DM patients the-11377C>G polymorphism of the adiponectin genewere association with brachial-ankle pulse wave velocity, the result suggested thatcarriers of the G allele gene have a higher risk of type2diabetes macrovasculardiseases. An elevation of baPWV is accompanied with descending adiponectin levelsin patients with type2diabetes mellitus of Fujian.
