Research Of First-line Treatment Of139Patients With Advanced Lung Adenocarcinoma

Lung cancer is the leading cause of cancer mortality worldwide. According tohistopathology, it is divided into non-small cell lung carcinoma (NSCLC) and small celllung carcinoma (SCLCN). NSCLC, accounting for about80%of all lung cancers,includes aquamous carcinoma, adenocarcinoma and undifferentiated large cellcarcinoma. Histology is representing an important variable in the decision making.According to histology, it is imperative to make individual treatment.Objective:To analysis the epidemiology、clinical characteristics and prognostic factors ofadvanced lung adenocarcinoma patients; To investigate the different therapy effects andtoxicity effects between PC、DC、GC chemotherapy first-line treatment for advancedlung adenocarcinoma.Methods:139advanced lung adenocarcinoma patients after therapy enrolled in the1sthospital affiliated of Dalian Medical University from Jan.2009to Feb.2011. Analysisthe factors like sex、age、smoking、metastasis sit; Collect advanced patients who usedcombined chemotherapy of PC、DC、GC. All of the patients had preponderated over2cycles.Study short time and long time effectiveness with ORR, DCR, OS, PFS,1-yearsurvival rate, and the adverse effects.Result:The univariate analysis suggested that smoking、 metastasis sit significantlyinfluenced survival of advanced lung adenocarcinoma.3kinds of chemotherapycombined with cisplatin, PC group’s ORR、DCR were33.8%、80.3%, it was higherthan others(P＜0.05). PC group’s PFS(months)、OS(months)、1-year survival rate were6.8、12.9，57.7%；DC group’s were5.8、12.5、44.7%；GC group’s were4.0、10.5、33.3%. PC group’s PFS is significant(P＜0.05). The major adverse effects were myelosuppression, nausea, vomiting. The hematological toxicity and gastrointestinalresponse in PC regimen group was significant lighter than others(P＜0.05).Conclusion:1. Male、smoker has high incidence rate of lung adenocarcinoma. Female’sproportion is rising. Smoking and metastasis sit are related with prognosis.2. PC and DC program have an advantage in the first-line treatment of lungadenocarcinoma. PC’s side effects are slightest among the3kinds of chemotherapycombined with cisplatin.According to individual patient, choose appropriate treatment.3. GC regimen has poor efficacy and its side effects are severe. Not recommendedfor first-line treatment for advanced lung adenocarcinoma...
