Objective:To evaluate immune function of partial splenic embolization (PSE) for hypersplenism in hepatocellular carcinoma (HCC) and Cirrhotic patients.Methods:From October2009to February2012, the No1Hospital of Shanxi Medical University intervention therapy ward selected45cases of HCC and cirrhotic patients with hypersplenism and divided the patients into2groups. One (PSE group) was treated with partial splenic embolization (PSE) combined with transcatheter arterial chemoembolization (TACE); the other group(TACE group) was treated with transcatheter arterial chemoembolization tace only.All patients were collected Peripheral blood cells before treatment and3days,7days,28days after the treatment to counts WBC, RBC, and PLT. Peripheral blood cells was also collected to detect ALT, AST, and TB1L level of the liver before treatment and7days,28days after it. Two groups of patients were all collected Peripheral blood cells before treatment and7days,28days after treatment to detect T cell subsets CD4+cell ratio,CD8+cell ratio and CD4+/CD8+ratio to compare these indicators between the groups and observe postoperative complications.Results:1. Peripheral blood cells:In PSE group, the WBC and PLT level of patients after surgery was significantly higher than preoperative levels (p<0.05), and RBC level changes were not significant (p>0.05); In TACE group, WBC and PLT significantly reduced (p <0.05), gradually recovered to the preoperative level28days after treatment, and RBC level changes were not significant.2.T cell subsets:In PSE group, patients’CD4+T cell ratio、CD4+/CD8+ratio rose with a significant difference (p<0.05), CD8+T cell ratio reduced with a significant difference before and after surgery (p<0.05); in TACE group, patients’CD8+cell ratio slightly increased with no significant difference (p>0.05), CD4+ratio and CD4+/CD8+ratio decreased slightly with no significant difference before and after surgery.3Liver function:in PSE group, patients’ALT and AST, TBIL changed non-significantly (p>0.05); in TACE group, patients’ALT, AST, TBIL rose significantly one week after treatment (p>0.05), and returned to preoperative levels28days after treatment. Conclusion:The peripheral blood cells, immune status and liver function have very important roles for comprehensive treatment of Hypersplenism in HCC and Cirrhotic Patients. TACE treatment can embolize the tumor and have a positive effect to improve their prognosis quality of life; PSE benefits the restoration and improvement of liver function because it can increase the number of peripheral blood cells, improve immune function, and is conducive to cell regeneration. Therefore, we are convinced that TACE combined with PSE treatment should be considered for treatment of Hypersplenism in HCC and Cirrhotic Patients...
