Part OneStudy on Surgical Operation Methods in Treatment of Pituitary Adenoma, a report of1319casesObjective:To investigate the clinical features of pituitary adenomas, surgical treatment strategies and treatment of complications.Methods:1319pituitary adenomas cases were retrospective analyzed the clinical character, evaluated the operation using the unilateral endonasal transsphenoid approach, keyhole craniotomy and direct craniotomy, and summarized the treatment of complications.Results:The incidence ratio of pituitary adenoma was1:1.34in male and female. The most common patient was from20to40years old, accounting for43.3%. There were836cases of invasive pituitary adenomas in this group (63.4%). Multiple hormone cell adenomas (26.4%) and prolactin hormone cell adenomas (21.7%) were the most pathology types. The symptom relief rate after surgery was95.6%, the cure rate was89.3%, the recurrence rate was7.6%, and the mortality rate was less than0.2%.Conclusions:The surgical treatment of pituitary adenomas can be used as the preferred solution, and the transsphenoid microsurgery approach is the most preferred operative approach. Part TwoAnalysis on Microsurgical Operation Using Unilateral Endonasal Transsphenoid approach in Treatment of Pituitary Adenoma, a report of1194casesObjective:To investigate the skill of microsurgical operation using unilateral endonasal transsphenoid approach in treatment of pituitary adenomas, therapeutic effect and treatment of complications.Methods:Operated using the unilateral endonasal transsphenoid approach,1194pituitary adenomas cases were retrospective analyzed the clinical character, endocrine condition, pathologic type, microsurgical skill, complications and follow-up.Results:In1037cases, the tumor was completely removed (86.9%). The symptom relief rate after surgery was90.2%. In1037follow-up cases, the cure rate was91.7%, the recurrence rate was4.7%,142cases had medicine or radiotherapy,38cases had performed secondary operation.Conclusions:For good outcome, low risk, and few complications, the microsurgical operation using unilateral endonasal transsphenoid approach is the most preferred operative method in treatment of pituitary adenomas.
