Research purpose:(1) To explore the value of conventional ultrasound combined ultrasound elastography imaging technology in the diagnosis of thyroid carcinoma.(2) Using binary Logistic regression model integrated analysed the respective features of thyroid nodule two-dimensional gray-scale ultrasound and color Doppler ultrasound and ultrasound elastography imaging, explored the value of respective features in diagnosis of thyroid carcinoma, then found out the valuable features and constructed binary Logistic regression model,in order to obtain a simple and effective method to improve the accuracy diagnosis of thyroid carcinoma.Materials and methods:85Patients with thyroid nodules examined by conventional ultrasound and ultrasonic elastography imaging from January2012to November2012were collected. Integrated ultrasound imaging prompted the possibility of thyroid carcinoma. All the thyroid nodules were confirmed by post-operation pathology. Had used a HITACHI Vision Preirus computer color-coded Doppler ultrasonic diagnosis apparatus which had ultrasonic elastography imaging function and a7-12MHz linear-array transducer. The patients laid on their back and were examined by direct contact.The thyroid lesion was first scanned under2-Dimensional sonography examination mode to get preliminary information,including its shape,edge,size,capsule,aspect ratio, echo intensity,calcification and lateral shadow.The color flow in the interior and periphery of the mass was observed under CDFI modes in sequence. Then we found the Scan section which had the most abundant blood flow signals.To calculated the number of blood vessels and with the spectrum of the brightest color Doppler sampling. Nature of flow signal was detected by spectral Doppler Ultrasonography Resistance Index and the Pulsatility Index were measure.At last,observed mass softness in elastography mode and given a score and completed the determination of the ratio of the strain rate. Save the pictures and record data in real time of the whole process.Result:(1) Pathology diagnosis of85thyroid nodules patients:Thyroid carcinoma were72cases,nodule goiter were5cases,Hashimoto’s thyroiditis were2cases,Subacute thyroiditis and thyroid adenoma with cystic lesions each were3cases.The conventional ultrasound combined with elastography imaging had higher accuracy84.71%(72/85) than conventional ultrasound’s74.12%(63/85) in the diagnosis for Thyroid carcinoma.(2) According to the output results of SPSS11.5, there were four independent variables selected on the final step of Logistic regression analysis, including:boundary,calcification of nodules, lateral shadow and ultrasound elastography. The model was:Logit(P)=-8.052+3.825x2+3.464x6+6.413x7+6.601x9,The likelihood ratio test was used to evaluate the fitting situation of the whole model. It was statistically significant.Conclusion:(1) This research had using Chi-square analysis methods, compared the accuracy in the diagnosis of thyroid carcinoma between conventional ultrasound combined elastography technical and conventional ultrasound, found the former had higher accuracy. Routine ultrasound examination was the basis of thyroid ultrasound, ultrasound elastography imaging examination contributed to the differentiation of benign and malignant thyroid nodules, it was better for the diagnosis of thyroid carcinoma if we combined the application of two methods.(2) The research using binary Logistic regression analysis method,integrated analyzed the9ultrasound diagnosis features of thyroid nodule,including its shape,boundary, calcification and elastic scoring,evaluated the application value of features in the diagnosis, finally the results showed that this method had found four significant characteristic variables of diagnosis,it had good clinical value for the diagnosis of thyroid carcinoma.(3) As a new type of imaging technology, ultrasonic elastography imaging technology had achieved good results in clinical,this technology was relatively mature and easy to learn,it provided more information for diagnosis and differential of benign and malignant thyroid nodules without increasing additional cost burdens, so it will be in favor of application.
