Objective1. To observe the changes of serum, endogenous Ghrelin and PYY in oral glucose tolerance test(OGTT)and analysis of correlations in patients of newly-diagnosed polycystic ovary syndrome(PCOS).2. To analysis the state of anxiety and depression in patients with polycystic ovary syndrome by self-rating anxiety scale (SAS) and self-rating depression scale(SDS).Methods1.48patients and32controls were selected and each group was divided into over-weight/obese and non-obese subgroups according to body mass index (BMI)≥25kg/m2or<25kg/m2. PCOS group was divided into PCOSobesity subgroup (26cases) and PCOSLean subgroup (22cases); and the control group was divided into CONobesity subgroup (17cases) and CONLuan subgroup (15cases).2. Collecting name, age, menstrual cycle and hair distribution such basic information of all the objectivs by questionnaire survey.Anthropometric measurements including height,weight,waist circumference(WC) and hip circumference were surveyed to calculate BMI and waist-hip ratio (WHR).Calculating SAS and SDS standard scores by SAS and SDS questionnaire survey.3. Blood samples were collected in follicular phase and were used for determinating the concentrations of luteinizing hormone (LH), follicle-stimulating hormone (FSH), testosterone (T), estradiol (E2), prolactin (PRL), progesterone (PRO), androstenedione (A2) and sex hormone binding globulin (SHBG) in all the subjects.4. All the subjects experenced an oral glucose tolerence test after8hours overnight fasted.Blood samples were collected in OGTT-0min and OGTT-30min and were uesd for determinating the concentrations of glucose, insulin, Ghrelin and PYY. Secreting function of islet beta cells and insulin sensitivity were assessed by HOAM-β and HOMA-IR.Results1. Comparisons of clinical parameters:BMI,WC,WHR,fasting insulin, HOMA-IR, HOMA-β, LH, LH/FSH、T and A2were higher in PCOS group than control group (P<0.05); but there was lower SHBG level in PCOS group than control group (P>0.05).BMI,WC,WHR,fasting insulin, HOMA-IR, HOMA-βwere higher in PCOSobesity subgroup than PCOSLean subgroup(P<0.05),but there was lower SHBG level in PCOSobesity subgroup than PCOSLean subgroup(P>0.05).2. Comparisons of Ghrelin and PYY:concentrations of endogenous serum Ghrelin and PYY in OGTT-0min,30min were lower than control group(P<0.05).Fasting levels of Ghrelin and PYY were lower in PCOSobesity subgroup than PCOSLean subgroup(P<0.05).Fasting concentration of Ghrelin had negative correlations with BMI,WC,WHR,fasting insulin, HOM-IR and HOMA-β in PCOS group(P<0.05); and fasting concentration of PYY had negative correlations with WC and BMI(P<0.05).3. Comparisons of SAS and SDS standard scores:there were higher SAS and SDS standard scores in PCOS group than control group, PCOSobesity subgroup than PCOSLean subgroup and infertility group than fertility group(P<0.05).Conclusions1. There existed significant obesity, insulin resistance and hyperandrogenemia in PCOS patients.2. Endogenous serum Ghrelin and PYY had abnormal reaction to OGTT in patients with PCOS:the concentrations of fasting serum Ghrelin and PYY were lower than controls, the declines of Ghrelin and rise of PYY were lower than the controls in OGTT, fasting concentration of Ghelin and PYY had negative correlations with BMI and WC. But the specific mechanism needs to be further studied.3. Obesity and infertile patients of PCOS had significant anxiety and depression.
