Objective:To analyze the prognosis of ventriculo-peritoneal (V-P) shunt in treating patients with traumatic brain injury who developed post-traumatic hydrocephalus (PTH) after decompressive craniectomy. To explore the factors that predict the efficacy of operation, so as to optimize the operation indication, improve the effectiveness of the V-P shunt in the treatment of PTH.Methods:A retrospective analysis of the clinical data of47cases of traumatic brain injury after decompressive craniectomy emergence of post-traumatic hydrocephalus in patients underwent V-P shunt. After1-2years follow-up, statistical analyze the prognostic factors.Results:According to the Salmon standard, grade3(symptoms improved significantly, the general situation is normal) in8cases, grade2(moderate symptoms improved, left nerve dysfunction) in10cases, grade1(less symptoms improved) in2cases, grade0(no change in symptoms) in23cases, grade-1(symptoms get worse) in3cases, grade-2(died in2months after surgery) in1case. Grade1-3are considered improved as the excellent group, while grade-2-0are considered unimprovement as the invalid group. Excellent group has20cases (42.5%),27cases in invalid group. Patients who have higher GCS score before V-P shunt, have better prognosis. Conclusion:Patients with PTH on traumatic brain injury after decompressive craniectomy, who have higher GCS score before V-P shunt, have better prognosis. Take GCS as the index for prediction the efficiency of V-P shunt operation has certain feasibility and clinical significance. Optimizing the operation indications, can improve the effectiveness of the V-P shunt.
