Objectives:To study clinical effect of resection of the mandibular benign tumor and reconstruction through intraoral approach.Method:In this article,15patients with benign mandibular tumor, who were admitted to the Departement of Oral and Maxillofacial Surgery, First Hospital of Zhejiang University, from June2010to March2013, were reviewed All of them were treated with benign mandibular tumor resection via intra-oral route and immediate reconstruction by bone grafts. There were5cases of ameloblastoma,9cases odontogenic cyst,1case of odontogenic myxoma. The pathological changes mostly located on mandibular body and mandibular angle, the highest of them was on the level of sigmoid notch. Six patients had preformed reconstructive titanium plate by the technology of CAD/CAM and the other nine cases of patients used reconstruction titanium plate directly intraorally. In most cases, the nerve was withheld by pushing it to the edge of the mandible. After the osteotomy, the pre-formed reconstruction titanium plate was fixed in the original site. According to the bone defect, we cut the sam size of the bone graft to repair.Because of the pathological change of one case of paient was located on the layer of sigmoid notch,so we made a auxiliary incision before the ear. Results:Thirteen cases of15had their Phase I healing. The patients with reserved inferior alveolar nerve had only temporary numbness in their lower lip. Two cases of15had a connection of bone graft and oral. They had their Phase II of the healing by putting idiom gauze. All of the patients after the therapy had a good result. The patients felt satisfaction about their appearance and had a normal morphology of the mandible. Conclusion:The resection of benign mandibular tumor and reconstruction could be done through intra-oral approach. Even it added difficulties to doctors during operation, but the obvious advantage is that it saves a long incision scar which could result in longtime mental affect to young people.
