Objective:Our research decide to investigate the expression and clinical significance of the human epidermal growth factor receptor type2(HER2/neu) and P53in patients with UPSC、EC、NE, and then to evaluate the correlation between them.To analyze the relations between them and the biological behaviour and clinical prognostic factors of UPSC, as well as the relation to approach the clinical significance in UPSC, and to discuss the expressions in UPSC diagnosis, illness evaluation and prognosis judge.Methods:Select cases that undergo surgery for endometrial carcinoma who were treated in Tianjin Central Hospital of Gynecology Obstetrics between June2004and December2012.40cases of UPSC, at the same time operation and the similar age of EC specimens of30cases, control group of30cases of NE.The expressions of both HER2/neu and P53were examined by immunohistochemical SP method. Computer pathological image analysis system was used to image analysis, combined with clinical data analysis the expression of the HER2/neu oncogene and P53and the relationship of UPSC occurrence, development and prognosis. Pearson chi-square test, and Spearman correlations statistical methods were utilized to analyze the expression of the HER2/neu and P53in UPSC and their relationship. Kaplan-Meier method was used for the survival analysis and made survival curves of the UPSC. Log-rank test was compared for survival curves. Multiple factors analysis by using the COX proportional hazards model.Results:1. HER2/neu staining was located on the tumor cell membrane. The expression of HER2/neu was all significantly higher in UPSC tissues than EC tissues and NE tissues (P<0.05), The expression of HER2/neu in EC tissues was higher than NE tissues, but the differences were no statistically significant(P>0.05).2. P53staining was located on the tumor cell nueleus. The expression of P53was all significantly higher in UPSC than EC tissues and NE tissues (P<0.05), The expression of P53in EC tissues was little higher than NE tissues, and the differences were no statistically significant(P>0.05). 3.The expression level of HER2/neu and P53in different tissue was positive correlated (r=0.392;P=0.012)。4. The result of the univariate analysis in UPSC tissues showed positive ascites or peritoneal washings cytology (P=0) was of prognostic value for overall survival(OS); vascular invasion (P=0.027), and positive ascites or peritoneal washings (P=0.008); greater omentum metastasis (P=0.019) for disease-free survival(DFS). Multiariable COX analysis shows, positive ascites or peritoneal washings is prognostic factors influencing patients DFS and OS (P<0.05).Conclusions:In UPSC,the expression of HER2/neu and P53was found., HER2/neu expression rate in patients of uterine papillary serous carcinoma is siginificantly higher than in patients of endometrial cancer and normal endometrium. Survival analysis demonstated that vascular invasion, and positive ascites/peritoneal washings;lymph node metastasis cytology and greater omentum metastasis were related to prognostic factors of UPSC.
