Study On Molecular Mechanism Of Naomaitong In Treating Cerebral Ischemia/Reperfusion Model Based On Metabonomics Method Of NMR

Objective:The material basis of TCM effect is the key to clarify the secrets of the TCM and to develop the indicators of scientific quality control. Stroke is one of the three harmful diseases to health, and its pathology and pathogenesis are complex. Naomaitong was proven to be a kind of effective TCM in treating stroke and relative disorders. But the mechanism of action is not clear yet. In recent years, the metabonomics mothed which reflects system biological ideology, has been widely applied in the study of pharmacology and toxicology, and we can research the medical treatment effect and the mechanism of action. The implementation of this subject not only can reveal the material basis of Naomaitong, but also can provide new methods and ideas for the establishment of the research of the compound TCM substances, which is based on the overall concept and the characteristics of TCM.Methods:(1) In this study, based on NMR of Metabonomics combined with PCA multivariate pattern recognizing technology, we gave the extracts of the compound Naomaitong. to the rats which had been divided into normal group, cerebral infarction model group, Nimodipine positive control group (6mg/kg), Naomaitong groups of low, moderate, and high doses (0.15、0.3、0.6g/kg), and the effect of extracts part of group of Naomaitong (0.3g/kg). And then we did the metabonomic research on the plasma and urine of these rats, analysed the interfers of Naomaitong treatment to the metabonomic networks, and the variations of relative metabolites.(2) At the same time, we testified the pathological index of HE staining and TTC staining, and visually observed the pathological changes of all the groups at cell level.Results:(1) According to the analysis of PCA and SPSS, the middle-dosage of Naomaitong can effectively inhibit the increase of glutamine, taurine, tyrosine, lactic acid in the plasma which caused by MCAO; and increase the levels of lipid, cholesterol, peculiariyies butyrate, N-acetyl aspartic acid, acetoacetate, choline, and return them to normal in the plasma of rats. The middle-dosage group and the effect of extracts part of group of Naomaitong can effectively inhibit the increase of threonine, glycin, trigonelline, glycerol, glutamine, N-methylnicotinic acid, citrulline, taurine in the urine which caused by MCAO; and increase the levels of creatinine, creatine, N-acetyl-(S-3hydroxybutyl)-L-cysteine,2-hydroxy-acetol, and return them to normal in the urine of rats.(2) The results of HE staining show that the cell gaps in the middle-dosage group of Naomaitong and the effect of extracts part of group of Naomaitong are smaller, and the neure endochylema and the nuclear membrane are clearer, most of the neure and neuroglia cells have intact structure and normal shape. The pathological changes of cell and intercellular edema are obviously lightened comparing with the model group, closed to the sham group. The results of TTC staining show that the the sizes of infarction in model group are larger than the other groups. The groups given exacts are improved in various extent, the middle-dosage group and the effect of extracts part of group, two of which, are smaller in the sizes of infarction. In the analysis of the variance of infarction areas, the sham group and medol group have very significant difference (P<0.001), and the middle-dosage group and the effect of extracts part of group also have very significant difference (P<0.001), which are have not obvious difference to the positive group (P>0.05). The middle-dosage group and the effect of extracts part of group of Naomaitong can reduce the sizes of infarction in brain tissue, with the curative effect similar to the Nimodipine group.Conclusion:The middle-dosage group of Naomaitong and the effect of extracts part of group can significantly decrease the pathologic damage of cerebral ischemia/reperfusion in rats, ameliorated the ischemia and anoxic state in brain tissue, and maintained the physiological balance of the brain tissue. This method establishs a new medical efficacy evaluation of compound TCM, and clarifies the function mechanism of Naomaitong in curing cerebral apoplexy as a whole.
