Background and Aim: Although,in recent years, due to the peroperative preciseassessements of liver function, advances in surgerical techniques and the increasingexperiences of surgeons, the hospital mortality rates in patients with the primaryhepatical carcinoma could be largely reduced, hepatic resection for HCC incirrhotic liver remains a challenging task,even in patients with Child-Pugh gradeA,the mortality and morbidity are still unsatisfactory. The different cirrhotic severitiesare stratified in mild, morderate,severe subgrades.the purpose of this study were toanalyse the results of different severity of cirrhotic liver resection in the past years andto propose safe strategies for cirrhotic liver resection.Methods: Between January2006and September2012, in our centre,171patientswith Child-pugh grade A are identified retrospectively analysed, theclinicalcopathological datas during the preoperative,interoperative and postoperativeterms are collected. the the subgrades of cirrhotic severity are identified beforemaking a decsion on operative strategy in hepatic cacinoma, the relationship betweenliver function and the morphological subgrades in cirrotic severity are staticallyanalysis, simultaneously, the risk factors for PHLF were subjected to univariableand multivariable analysis.Reseult: The severity of liver cirrhosis correlates with ascites, varis and platelets,which has a statistical significance. The incidences of post-hepatectomycomplications and liver failure are respectively32.2%,15.2%, and perioperativemortality is0. Univariate and multivariate analysis demonstrate that the relationshipof liver cirrhosis with of postoperative liver failure and complications are statistically significant(p<0.05).Conclusion: In patients with Child-Pugh A grade, liver cirrhosis is a independentprognostic factor of postoperative liver failure and complications.
