OBJECTIVE:On the base of pathological results,by comparing the MSCT morphology and metabolism of18F-FDG PET/CT features in67cases of pulmonary diagnosis of space-occupying lesion of differences, explore MSCT morphology,18F-FDG PET/CT functional metabolism and a combination of the value of qualitative diagnosis of space-occupying lesion to the lungs.MATERIALS AND METHODS:Retrospectively collected67cases with pulmonary space-occupying lesions(27female,40male,age range18-76y,mean age56y) which were collected from March1999to December2012in Wuhan Union Hospital.All cases were complete with18F-FDG PET/CT and CT examination data and also had pathology confirmed according surgery or positioning biopsy under CT guidance(16benign,51malignant).Using MSCT, SUVmax, PET/CT and MSCT SUVmax4kind of diagnostic method to evaluate all the cases.Image evaluation:①MSCT method based on cross sectional thin layer image and MPR image morphology were evaluated for lesions;②the SUVmax method used2.5as the cut-off point to identify benign and malignant lesions;③PET/CT image fusion method based on evaluate the pathological changes of morphology and function of metabolism;④MSCT combined SUVmax method considering both the diagnosis.Four kinds of methods of evaluation result compared with pathological results, calculate their respective diagnostic sensitivity, specific, positive predictive value, negative predictive value, YI, and the OP; Compare four kinds of method for overall and different size, different density of pulmonary space-occupying lesions. By pairing sample t test to compare the diagnostic accuracyRESULT:MSCT morphology combined with18F-FDG PET/CT diagnosis of space-occupying lesions of the lung function metabolism sensitivity, specific, positive predictive value and negative predictive value, YI and OP in turn90.2%,75.0%,9.0%,75.0%,0.652,28.The accuracy of MSCT combined SUVmax to diagnosis the overall cases was higher than MSCT morphology, SUVmax and PET/CT fusion image (P<0.004、P<0.006、P<0.024);the maximum diameter of3cm or less lesions, the diagnosis accuracy was higher than SUVmax and PET/CT fusion images (P<0.032,P<0.023);also the diagnosis accuracy of unsolid or partly unsolid lesions, the diagnosis accuracy was higher than single SUVmax method (P<0.042)CONCLUSION:MSCT morphology combined with18F-FDG PET/CT function metabolism diagnosis of pulmonary space-occupying lesions has the very good application value.Combination of morphological and functional metabolism, provide us with more information, has greatly increased the diagnostic sensitivity, specific, positive predictive value and negative predictive value. For smaller (the largest diameter3cm or less) and unsolid or partly unsolid lesions.MSCT morphology and function of PET/CT combined metabolic significance is bigger, and morphological information is particularly important.
