Objective: the present study was designed to search the meaning of screw Auxiliary inthe treatment of dry unstable tibial fracture with two types of Internal fixationMethods: Sixty cases of tibial shaft fracture instability, treated from January2007toSeptember2012,were randomly divided into two groups, each group of thirty cases,onegroup were treated with Steel combination lock auxiliary fixing screws (L+L group), inthe groups, men sixteen cases, females fourteen cases, Age Thirty-six±five years of age,according to AO classification,fracture type A1nine cases, A2seven cases, B1eight cases,type B2four cases, C2type C2two cases;and another were treated with the fixation ofintramedullary nail combined with screws (P+L group),in the groups, men seventeencases, females thirteen cases, Age thirty-four±six years of age, according to AOclassification,fracture type A1thirteen cases, A2eight cases, B1six cases, type B2fivecases, type C2two cases.。observed two measured blood loss cases, operative time,healing time,and complications.Results: All patients were followed up for eighe months to two years and six months, anaverage of ten±two months, compared blood loss cases and operation time, P <0.05, twogroups was statistically significant;in the P+L groups,fracture healing time was fromeight months to eighteen months: the average healing time was8.0±2.6months:; in the L+L groups, fracture healing time was from six months to twenty-four months: theaverage healing time was8.05±2.4; t test inspected two independent samples,P>0.05,the two groups was not statistically significant.。 according to referenceJohner-Wruhsevaluation criteria[1], in the L+L groups, the efficacy of twenty-two cases were excellent,seven cases were good, good rate was93.33%; in the P+L groups,twenty-four caseswere excellent,five5cases were good, one case was suit,excellent rate of96,67%;compared two groups, P>0.05, two groups was not statistically significant.in the L+L group,one case occurred plate fracture, the complication rate was3.3%,the P+L grouphad no nail breakage fracture、fracture delayed healing and other complications.Conclusion: Conclusion: The two methods were both excellent internal fixations for thetreatment of unstable tibial fracture;For the patients with severe comminution, inparticular the patients need screw to reconstruc bone stability with multiple planar andmulti-angle, Steel combination lock auxiliary fixing screws was a better indication;For thetibia fractures, the fixation of intramedullary nail combined with screws was a betterindication.
