| Objective:As an important reform of the medical and health system, national essential drugsystem is a new system aimed at improving the health of the people continuously,meeting the needs of the public and promoting the basic health care system coveringboth urban and rural residents accessible to basic health care. At present, the initialestablishment of a national essential drug system has been completed, theimplementation of national essential drug system work entered a consolidation stage.Among them, the consolidation and improvement of the basic drug supply securitywork is one of the key difficulties. But the drug centralized bidding and purchasing isa key component and bettering this system is even more crucial especially in ensuringthe supply of basic drugs at a reasonable price and providing safe and effectivemeasures.Through a detailed analysis of the "Double Envelope" Mode and "CheckPrice Mode of Basic" Mode in Jilin province, this paper summarizes the experiencepractice effectively at the same time, analyzes the problems and provides thecountermeasures to further complete the system to ensure the supply of basic drugs,and presents some suggestions.Methods:1. Sources of Data: Data collected in2010and2011year is from the "drugbidding system in Jilin Province pharmaceutical procurement service platform",inclusive of the implementation of the programme, documents and bid, the relateddata of basic drugs basic medical and health institutions for centralized procurementcompared with data from Liaoning province and Heilongjiang province in2011. Thepaper also analyzes the drug varieties as well as the price differences.2. The Questionnaire Survey: Research objects are from95basic medical andhealth institutions,25counties (cities, areas) and10Jilin provincial cities as well as900urban and rural residents who have enjoyed the rights of the basic medical service at least once. The survey mentions, the drug prices and residents’ satisfaction.Results:1. Basic drugs "Double Envelope" Mode of centralized bidding and purchasingthrough procurement directory not only standardizes the Bidding forms, specificationsbut also avoids the qualified blind purchase malpractice of "Check Price" Mode.2. In the "Double Envelope" Mode, some large scale enterprises, which scoremuch higher in technical standard occupy the majority so as to ensure the quality ofdrugs. Among them, the number of drug products from the Ministry of Industry andInformation Technology companies ranking the top100enterprises is523accountingfor37.14%; the number of the patented drugs and drugs from theNational-Science-and-Technology-Progress-Award winning companies is91accounting for6.46%and the number of separate pricing, quality and price ofproprietary Chinese medicines, protection of varieties of traditional Chinese medicineis99accounting for7.03%.3. The drug price of "Double Envelope" Mode drops sharply. Compared with theprice of the drug products from those Check Price Mode companies, the lower priceproducts account for91.65%; compared with the national highest retail price, theaverage has dropped to51.71%; compared with the price released by the NationalDevelopment and Reform Commission, the average declines18.93%; the price isabout17.2%lower than Heilongjiang Province and8.4%lower than LiaoningProvince on the average.4.82.82%of those survey objects think that the basic medical and healthinstitutions drug price has reduced;62.66%believe that seeing a doctor is muchcheaper in basic medical and health institutions;82.09%hold that drugs in basicmedical and health institutions are cheaper and74.71%prefer to see a doctor in themedical and health institutions.5. There is a price raise in85products among the overall1138productsincluding30varieties of exclusive products,55of the traditional Chinese medicineand30of Western medicine.6. Based on the result of the analysis,632products, after bidding and bargaining,are still in short supply among1828procurement directory drugs in this area and theflow rate was34.57%. Conclusions：1."Double Envelope" Mode of centralized bidding and purchasing clarifies theprocurement catalog, adheres to quality first and forms an effective quality evaluationsystem.2."Double Envelope" Mode of centralized bidding and purchasing unifies andreduces the price of drugs effectively and the cost of the basic medical and healthservices.3. On the "Double Envelope" Mode of centralized bidding and purchasingplatform, the price of some exclusive drugs and Chinese traditional medicine is stillon the high side. As a result, further study is required to solve the problem of falsehigh price of drugs and to guarantee clinical medication needs.4."Double Envelope" Mode system of centralized bidding and purchasing is notperfect as in no bidding and bidding at the pharmaceutical part procurement catalogueresulting in shortage of drugs used in medical institutions.Suggestions:1. Nation-wide basic drug bidding procurement policies must be made andimplemented to ensure the implementation of essential national drug pricing,fixed-point production and a unified procurement and distribution.2. The government shall fortify the classified procurement, explore variousforms to improve the basic drug Centralized Bidding and Mode.3. The price of drugs must be displayed clearly. Some drugs must be producedunder severe inspection. The exclusive drug prices must be lowered and drug supplysecurity must be guaranteed.4. Dynamic adjustment of the drug price must be made. Service of improving theunified distribution, unified settlement and guaranteeing the basic drug supply mustbe perfected. |
