Objective:To evaluate the finding of1H-MRS and to explore the correlationbetween the finding of1H-MRS and the pathological grading of gliomas.Method:Sixty-three patients with cerebral glioma confirmed by surgery andpathological results between January2012and December2012were selected asresearch subjects. All patients prior to surgery underwent CT and MRI scans with1H—MRS on GE3.0T magnetic resonance system. The spectral line in the solid area ofglioma and normal area of the opposite side were observed with1H—MRS.Postoperative surgery to remove the tumor and part of the tumor surrounding tissuewere fixed in formalin solution, production of tumor specimens24hours later. After48hours, specimens were taken dehydration, transparent, wax dipped, embedded, sliced,HE staining. Nuclear atypia, mitotic index, presence or absence of necrosis wasobserved under an optical microscope, then patients were divided into two groupsreference to the2007WHO diagnostic classification of tumors of the central nervoussystem standards. Ⅰ~Ⅱ for low grade gliomas,24cases, Ⅲ~Ⅳ for high gradegliomas,39cases. Select the normal brain tissue as a control group, there weresignificant differences in NAA, Lac, Cr, Cho, Lip levels. Calculated Cho/NAA, Cho/Cr, NAA/Cr, then use the SPSS13.0statistical software for statistical analysis.Results:1H-MRS detection of brain glioma is mainly as follows: a significantreduction of NAA peak, moderate decline of Cr peaks and significantly increased ofCho peak, the corresponding NAA/Cr and NAA/Cho ratio, Cho/Cr ratio increasedlead to descreased of NAA peak, which is the result of normal nerve tissue occurredglioblastoma tumor invasion and replaced. Brain neurons reduced, functionalimpairment, Cho levels higher in white matter than gray matter, but when gliosis orneurodegeneration, Cho concentration thereupon rising. The Lac peak in individualcases showed that lactate in gliomas, the normal aerobic respiratory chain damaged, anaerobic glycolysis increased and brain tissue and cell apoptosis. Glioma tumor realterms compared with the contralateral normal brain tissue, its typical spectroscopymainly manifested that: significantly reduced of NAA peak, significantly increased ofCho peak, not changed significantly decreased of Cr peak,35patients appeared thelarge Lac peak. Cho/NAA and Cho/Cr ratio increased significantly, then, NAA/Crratio decreased significantly. Statistical analysis, the difference was statisticallysignificant (P <0.05), the ratio of metabolites in the different levels of glioma weredifferent. Low grade group: Cho/NAA2.15±1.07, Cho/Cr3.21±1.24; high gradegroup: Cho/NAA9.31±3.17, Cho/Cr4.83±1.92,.T-test analysis, the differencebetween the two groups of data was significant (P <0.05). In addition, the majority ofpatients in the high-grade group had clemency Lac peak and Lip peaks.Conclusion:Glioma1H-MRS contributes to the clinical diagnosis of glioma, and itwas the basis for preoperative tumor grade, treatment and prognosis.
