Electroacupuncture pretreatment is proposed in recent years as a new method to reduce the target organ ischemia-reperfusion injury which based on the theory of traditional acupuncture, apply dual stimulatation to the corresponding acupoint by passing the electricity current very close to the human bio-electircity through needle in order to achieve the purpose of disease prevention as well as treatment. The current study has proved electroacupuncture pretreatment induced ischemic tolerance effects in the heart, spinal cord, brain and other organs. However, there is no report has indicated that electroacupuncture pretreatment has a similar protective effect in the kidney. In this circumstance,"Shenshu" accupoint which is closely related to kidney urinary system function is chosen to be the experimental select for the study. By using of repeated electroacupuncture established on rats renal ischemia reperfusion injury model, renal function, cell proliferation, apoptosis and regulation of related gene point, research pretreatment with repeated electroacupuncture on renal ischemia-reperfusion injury in rats and related molecular mechanisms. Objective:To study the impact of electroacupuncture preconditioning in renal ischemia reperfusion injury as well as it is impact on renal tubular epithelial cell apoptosis. To explore the molecular mechanisms in reference to the electroacupuncture (EA) at "Shenshu" acupoint against renal ischemia-reperfusion injury in rats.MethodsFifty SD male rats were randomly divided into4groups, five male rats were taken as sham operation group with only bilateral renal pedicle exposing for45minutes. Another3groups (n=15):I/R group, PB+I/R group and EA+I/R group. Renal I/R model was established by bilateral renal pedicle clamping for45minutes and reperfusion, animals in PB group were in traperitoneally injected with pentobarbital sodium40mg/kg/day for5days before I/R, animals in EA group received electroacupuncture at the "Shenshu" acupiont30min/day for5days under pentobarbital sodium anesthesia before I/R. Animals were sacrificed at each interval of1d,3d,7d after the last treatment, then The levels of Blood Urea Nitrogen and Creatinine in serum were assayed. The Histological Score of Kidney was evaluated. The cell Apoptotic Index was examined by means of TUNEL. The expressions of PCNA, bcl-2, bax, Fas and FasL were measured by means of immunohistochemistry.ResultsThere was no significant difference among I/R and PB+I/R groups. Compared with PB+I/R groups, BUN, Scr in EA+I/R group were significantly lower in3d and7day after Renal I/R model was established. The AI, Bax, Fas, Fasl in EA+I/R group were decreased and the PCNA, Bcl-2in EA+I/R group were increased obviously in3d after Renal I/R model was established. Morphology changes were examined under light microscope and electronmicroscope. In the I/R and PB+I/R groups, interstitial edema, brush border disappearance ofresidual renal tubular epithelial cells, desquamation and necrosis of a large numberof epithelial were observed in renal tissues. Mitochondria disappeared,endoplasmic reticulum reduced and chromatin degeneration were examined underelectron microscope. brush border lost of some tubular epithelial cells and few particlecasts were observed in renal tissues. Relative ectasia of cell organelles and nuclearchromatin margination were examined under electron microscope. Compared with the IR group, themorphology changes under light microscope and electron microscope of EA+I/R group were reduced and the tubular injury score of the EA+I/R group were decreased remarkably.The immunohistochemical staining results showed that compared with PB+I/R groups, the AI, Bax, Fas, Fasl in EA+I/R group were decreased and the PCNA,Bcl-2in EA+I/R group were increased obviously in3d after Renal I/R model was established(P<0.05)..ConclusionThrough this research result can be concluded that:(1) the repeated electroacupuncture shenshu point pretreatment can reduce renal ischemia-reperfusion injury in rats, reduce the serum levels of Scr, BUN, effectively protect the renal function. Its mechanism and promoting renal tubular epithelial cell proliferation and apoptosis.(2) promote repeated electroacupuncture shenshu point pretreatment of PCNA positive cells in rat renal tubules, significantly increase the number of that Chinese yam rhizome lavage pretreatment can promote the regeneration of the damaged kidney repair.(3) Repeated electroacupuncture stimulation Shenshu pretreatment renal tubular epithelial cells to a significant increase in anti-apoptotic gene Bcl-2protein, Bax, Fas and FasL protein expression decreased, suggesting that these genes may be involved in the repeated electroacupuncture stimulation Shenshutubular epithelial apoptosis regulation hole preconditioning reduces I/R injury, its specific mechanism needs further research.Kidney ischemic postconditioning has an inhibitory action on apoptosis of renal epithelial cells. It play the role mainly through trigger Fas and Fasl of exogenous death receptor-dependent apoptosis pathway and triggers Bcl-2and Bax of endogenous mitochondrial apoptosis pathway. Compared with ischemia reperfusion injury, exogenous apoptosis pathway still play a relatively weak role in promoting apoptosis, but endogenous apoptosis pathway mainly play the role of apoptosis inhibition.
