Objective: Through collected the hepatic veno-occlusive disease (HVOD),budd-chiari syndrome (BCS,10cases), posthepatitic cirrhosis (20cases), measured theportal vein diameter, to determine whether there is a significant statistics differencebetween the three groups. Analyzed the CT image features of10patients who hasHVOD, then to compared the3D-CT image portal vein reconstruction with the digitalsubtraction angiography(DSA), and to discussed the application value of CT scan andits post-processing reconstruction technique in differential diagnosis of HVOD.Materials and Methods: Retrospectively analyzed the clinical and radiologicalmaterial of3groups of patients (10cases of HVOD,10cases of BCS and20cases ofposthepatitic cirrhosis).①. To observed the CT scan and enhancing image of the3groups, including the imaging characteristics of3D-CT reconstruction, and the situationof collateral circulation of3groups diseases; then to compared the images of3D-CTreconstruction with DSA images.②.To compared the main portal vein diameter withthe normal value of portal vein of HVOD patients;③. UsingAVONA comparing3groups of patient, including the main portal vein and its branch of left and right, thediameter of splenic vein,observing whether there is statistically significant;④. Tocollected30cases of CT and DSA patients, and to observed whether there is anyrelationship between portal vein which is measured by CT and portal venous pressurewhich is measured by DSA.Results:①.In the vein and collateral circulation aspect, the3D-CT reconstructionis correspond with DSA images; In the aspect of hepatic vein images, DSA is superiorto CT. The information of collateral circulation: HVOD group:1.2piece; BCS group:2.5piece; posthepatitic cirrhosis group:2.1piece.②. The HVOD patient’s averagevalue of mian portal vein is slightly lower than normal value.③. The left and right ofportal vein: no any statistical differences between HVOD and BCS(LSD-test). The mainportal vein, the left and right of portal vein and splenic vein: compare those3groups of diseases found there is statistical differences. There was significant different betweentwo group (P<0.05);④. No any relationship between portal vein which is measured byCT and portal venous pressure which is measured by DSA.Conclusion: Firstly: The enhanced CT scan could identify the typical HVOD, BCSpatients and the posthepatitic cirrhosis, while the3D-CT reconstruction technical coulddo further identify on HVOD and BCS; Secondly:3D-CT reconstruction of portal veindiameter, collateral circulation conditions,etc, which is correspond with DSA imagescan make auxiliary diagnosis for the disease.The last:Learning more about medicalhistory, clinical manifestation, laboratory examination and other auxiliary examinations,etc, are also plays a very important role in the differential diagnosis. In a word, CTexamination has certain clinical value in differential diagnosis of HVOD, BSC, andposthepatitic cirrhosis.
