The Attitude On Hospice Care And Analysis On Influencing Factors Of Cancer Patients And Their Families, Medical Personnel And Hospital Administrators

Objective:With the increase of cancer patients and old population, the demand of hospice care increases quickly. This study aimed to examine the willingness of hospice care work of medical personnel, the attitude of establishing hospice care sickroom or division of hospital administrators, the willingness of receiving hospice care service of dying patients and their relatives; and to analyze the associated factors of the attitudes for hospice care among four types of population.Methods:A cross-sectional survey was conducted in five cities, including Jiamusi city in Heilongjiang, Hengyang city and Changde city in Hunan, Guilin city in Guangxi and Huludao city in Liaoning. Demographic characteristics and the attitude on hospice care were surveyed. Dataset was created using the EPIDATA3.02package, and the SPSS13.0package was used to analyze the data. Frequencies and percentages were used to describe the distribution of data, χ2test and fisher exact test were used to compare the proportions of surveyed persons accepting hospice care. Binary logistic regression was performed to estimate the determinants of hospice care.’α=0.05’was selected as the significance level of statistical test.Results:1. The acceptance rate for hospice care was30.8%(95%CI:26.9%-35.0%) in a total of497cancer patients. Univariate analysis showed that education, occupation, income, health insurance, and religious beliefs were the impact factors.2. The acceptance rate for hospice care was68.4%(95%CI:64.2%-72.3%) in a total of509patient families. Univariate analysis showed that age, marital status, relationship, income, and ever hearing about hospice care were the influencing factors.3. The acceptance rate for working with hospice care was56.6%(95%CI:52.0%-61.3%) in a total of463health care workers. Univariate analysis showed that age, education, occupation, working lifetime, having critically ill patient at home or not were the influencing factors.4. The acceptance rate for opening hospice care division was89.9%(95%CI:82.2%-93.5%) in a total of119hospital administrator. Univariate analysis showed that working lifetime was a significant influencing factor.5.97.9%of hospital administrators thought that governmental attention and capital investment were inadequate;97.6%thought that the workforces of hospice care was not enough;96.5%thought that the workers’knowledge and skills cannot meet the minimum requirement; and90%of them worried that opening hospice care room would induce more doctor-patient accidents.Conclusions:1. The acceptance rate of cancer patients for hospice care was30.8%(95%CI:26.9%-35.0%), that of patients’family members was68.4%(95%CI:64.2%-72.3%), and that of hospital workers for hospice care was56.6%(95%CI:52.0%-61.3%).89.9%of hospital administrators agreed to build room or division (95%CI:82.2%-93.5%).2. Cancer patients who are civil servants and retire cadre, earn more than1500Yuan per month, and have religion, are more likely to accept hospice care Patients’family members who are young, and are children of patients, earn money ranging from1500to4500Yuan per month, and have heard of hospice care, are more possible to accept it. Hospital administrators with more than5years’work experience prefer for hospice care. Doctors and nurses who are35years and older, received higher education, unknown about this career are more likely to choose hospice care as their career.
