BACKGROUND and AIM:Allogeneic hematopoietic stem cell transplantation is one of the important measures in the treatment of hematopathy。But graft-versus-host disease is a major complication of the measure。It is the most important factor lead to transplant-related mortality and graft failure. Because of its diverse clinical manifestations and varying severity, multi-factor is the focus of current research and difficult point. A large number of studies have confirmed that immune cells release inflammatory factors, such as IL-2, TNF-a, play a key role in the pathogenesis of aGVHD. Recent studies have shown that HMGB1is released to the extracellular also be used as a pro-inflammatory cytokines involved in inflammatory reactions occur. The current lack of correlation studies for aGVHD and HMGB1. The experimental spirit to explore the correlation. Detectting occurring aGVHD patients and without aGVHD patients serum levels of HMGB1.METHODS:By enzyme-linked immunosorbent assay (ELISA) to detect18cases of allogeneic hematopoietic stem cell transplant patients before and after transplantation serum HMGB1,IL-2 level,respectively.The detection time is before pretreatment(-14d), after pretreatment(-1d),+1d,+7d,+14d,+21d,+30d for, with mean and standard deviation indicate.Compared to determine IL-2, of HMGB1with aGVHD relationship and correlation of IL-2and HMGB1. By RT-PCR method to further detect the aGVHD patients without aGVHD patients HMGB1gene expression differences.RESULTS1) pretreatment (with or without TBI), hematopoietic stem cell infusion for serum IL-2and the release of HMGB1had no significant difference.2) Serum IL-2increased before the onset of clinical symptoms and positively correlated with lymphocyte proliferation.3) aGVHD patients with serum IL-2level increased in+7day and+14d reached the peak. IL-2serum levels parallel to clinical treatment when clinical symptom control in patients IL-2serum levels decline always.4) aGVHD patients with serum HMGB1level increased in+7day and+14d reached the peak. HMGB1serum levels parallel to clinical treatment when clinical symptom control in patients HMGB1serum levels decline always.5) The serum levels of HMGB1and IL-2were positively correlated, and consistent with the clinical symptoms of aGVHD. CONCLUSIONThe patients occurring aGVHD with higher expression of IL-2and HMGB1than the patients without aGVHD. HMGB1correlate with severity of aGVHD.It can be used as a diagnostic marker of aGVHD.The aGVHD symptom control, HMGB1expression level decreased,indicate that HMGB1can be used as a detion marker of the result of the treatment.
