| Objective:To analyze retrospectively the followed-up data of discectomy in thetreatment of lumbar disc herniation of38cases who was followed up more than tenyears.To evaluate the clinical curative effectã€reoperation rate and investigate theinfluence of curative effect.Methods: We analyze retrospectively38patients who accept discectomy in thetreatment of lumbar disc herniation and whose operation time from January1997toDecember2003. We glean the information of38patients including gender,age, chiefcomplaints,physical examination,smoking history,lumbar sprain history and basicdisease etc. The long-term surgical clinical effects was evaluated withStauffer-Coventry,.We also evaluate preoperative and followed-up scores of VASã€JOA and ODI and record postoperative recovery condition and self evaluation.Lumbar X-ray will help to analyze the changes of the operation sections and adjacentsegment.Results:The followed-up time was from eleven to seventeen years,the averagefollowed-up period was13.4years.The excellentã€goodã€fairã€poor rate of followed-upclinical curative effect was26.3%ã€42.1%ã€18.4%ã€13.1%,reoperation rate was13.1%.JOA scores of preoperation and followed-up was7.05±4.183and21.51±6.035, ODIscores of preoperation and followed-up was35.86±9.813and9.38±7.956, VASscores0f leg pain and low back pain of preoperation and followed-up was7.49±0.961and3.05±2.285分. There were statistically significant differences in JOAã€ODIã€low back pain and leg pain VAS scores between preoperative and followed-up (P<0.05).The imaging changes: all the lumbar X-ray of15cases showed thatoperation sections have different degrees of narrowing, but all the operation sectionsdid not appear instable performance.Conclusion:(1)The long-term excellent or good rate of clinical curative effect ofdiscectomy in the treatment of lumbar disc herniation was, the majority of patientswith low back pain can significantly relieve symptom. Postoperative intervertebralheight Narrowing did not necessarily cause lumbar instability. Discectomy in thetreatment of lumbar disc herniation is still the first choice.(2) Long-term fatigue,lumbar trauma history can affect long-term clinical curative effect. |
